People Powered ‘My health, support and care are directed by me, co-ordinated, and works well together’ I’ll no longer be a patient or a client – I’ll be a person I can plan ahead and keep control at times of crises We will support people at times of crises to have the right support as soon as possible, to enable people to return home and to their communities I am able to get skilled advice to plan my care and support We will enable people to promote their own health and wellbeing supported by self care and self management I have considerate support delivered by competent people We will develop the infrastructure to deliver truly co-ordinated care and support

‘Living well in communities’ Closer to Home ‘Living well in communities’ Users have more say over their care. They are supported to stay in their own homes and within their own communities. GPs are at the centre of care co-ordination. Care is more convenient with fewer hospital stays Systems are joined up and enable service provision, funding flows to where it is most needed within a community. Care is delivered quickly and effectively and duplication is avoided.
