Science Fair Entry Form Name: Date: Science Fair project title/Question: ___________________________________________________________________________________________________________________________________________________________________________________________________ Brief Description of project: include how you plan to conduct your experiment in order to answer/test your hypothesis, materials needed, a brief description of your planned procedure and how you will gather your data. _________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________ List any special needs for your project: Electrical outlet/access to water, etc. _________________________ _________________________________________________________________ Parent Signature:______________________________________________ Teacher Signature:_____________________________________________

Science Fair Written Report Your written report must be placed in a folder and presented to your Teacher. This must be written in cursive or typed. Please include a cover page with your name and project title. Be sure to include copies of pictures, charts, tables and graphs in your report. Your written report must include the following: Introduction: Describe your purpose for doing the project. Include your question and the problem you set out to find the answer to. Tell if your experiment discovered new science or can it help our lives or change what we already know to be true. Hypothesis: What did you predict would happen and why? Equipment and materials used: Be specific and give details. Procedure: Explain step by step how you conducted the experiment. Results: What happened each time you did your experiment include graphs/charts and tables here. Conclusion: What I learned from my experiment.

Project Question Think of a question: Your question will drive your entire project. Make sure your question is something that can be measured with data and can be tested using the Scientific Method. You may use the project question as the title for your project. Example: How will the length of wire affect the brightness of a lightbulb? (This is a good question because it can be tested and you can gather data.) Poor example: What is the moon made of? (You can easily find this answer through research.) My project question: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

Project Hypothesis State Your Hypothesis: Based your research or prior knowledge and experience decide what do you think the outcome will be of your experiment. Make a good guess as to what you think will be the answer to your question. Also, explain WHY you think that will be the outcome. Remember it is okay if your answer is not correct. Make sure your hypothesis is in a complete sentence. Example: The length wire will affect how brightly the lightbulb shines. I think that the shorter the distance the electricity has to travel the brighter the light will be. My Hypothesis: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

Project Experiment Describe your experiment: Clearly write out the procedure you followed. Remember your project must follow the Scientific Method and that there needs to be only one variable. Give a step by step clear restatement of what you did. Use another sheet to add more steps. Step 1: _________________________________________________________________________________________________________________________________________________________________________________________________________________________________ Step 2: Step 3: _______________________________________________________________________________________________________________________________________ __________________________________________________________________________________________

Project Observations and Data Conduct experiment and Gather Data: You will need to conduct your experiment a minimum of three times in order to get accurate data. Be sure to log your data carefully each time. You will need numerical data. Numerical data can be temperatures, distance, height, etc. Create a chart to help organize your data you will analyze it later so this is important. Also, write down any observations or unique findings as you go along. You may use a log or notebook paper to record all your data. Data and Observation notes: _________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________ ___________________________________________________________________________________________________________________________________________________________________________________________________________________________________________ ______________________________________________________________________________________________

Project Results Determine the Results: Now is time to review and talk about your results. Explain what happened during your experiment and be sure to analyze your charts and graphs. Write out the results of each test in your experiment in paragraph form using complete sentences. Make sure you include the numerical data as well as other important observations you made. Results: _______________________________________________________________________________________________________________________________________ _________________________________________________________________________________________________________________________________________________________________________________________________________________________________ ____________________________________________________________________________________________________________________________________________________________________________________

Project Conclusions Draw Conclusions: Analyze the results and determine how the results help you answer your project question. Write your conclusion in a complete sentence using your question to begin your answer. You will need to tell why your hypothesis was correct, or if the results contradict your hypothesis. If it was not supported explain why you think so. End this paragraph by stating how you would change or improve your experiment in the future. Answer Question: _______________________________________________________________________________________________________________________________________________________________________________ Did results support hypothesis?: _________________________________________________________________________________________________________ How would you improve or change experiment?
