Obtain consent of the parent/child for the referral and record, unless gaining consent puts the child at further risk of significant harm. e.g. when the professional has concerns relating to sexual abuse or Fabricated illness. Listen to and record all information Document fully your concerns, observations, discussions, decisions made and by whom the referral was taken. Do not question the child or try to obtain further disclosure. Do not promise confidentiality but advise you must share information to ensure the child is protected. Confirm the referral in writing via a multi-agency assessment and referral form (CAF form) within 48 hours.
