Person Centered Planning

What is It? Planning tool New Perspective Starts with the person not with the system: Who am I? Strengths based Individualized Brings together people who care about the person To look at an individual in a different way. To assist the focus person in gaining control over their own life. To increase opportunities for participation in the community. To recognize individual desires, interests, and dreams. Through team effort, develop a plan to turn dreams into reality.

Are there Different Kinds of Approaches? Who should be involved? Family Friends Peers Co-workers Neighbors Professionals Peers; Friends from school Co-workers--- The people have a clear and shared appreciation of the talents and capacities of the focus person. People have a common understanding of what the focus person wants. The group involved agree to meet regularly to review activities. The group include a strong advocate or family member assuring that the interest of the focus person is being met. That the group include a person committed to making connection to the local community. Professionals: Caregivers, teachers, therapists, paras etc…Who: People who know you well, know what you like/don’t like/know your goals, your concerns, about your background, How to ask people to participate- give opportunity to say “yes” Everyone is so busy

The Role of The Facilitator Plans the process Guides the Group through the process Makes sure all voices are heard Creates a positive atmosphere Follows up Good Facilitaotors– hold the core PCP values– believe in the person Good listener Respects different points of view Remains neutral (sometimes someone who doesn’t know the person is good) Curious about the person and process Good communicator– prepared/follows up Makes sure everything is captured Opened ended questions Keeps things moving Creates a doable action plan Helps move through roadblocks

Identifies strengths, gifts and abilities Identifies challenges and Concerns Identifies the persons desires Identifies Community/Natural Supports Identifies Resources: Natural and Paid Identifies Supports needed Identifies Goals: What do I want? Identifies Next Steps: Who, what, when and where? Puts the Plan into ACTION! What are my hopes and dreams? What brings me joy? What is important to me? What are my gifts? What are my strengths and abilities? What am I good at? What are my challenges? What are my worries and concerns? What do I need?
