Accessing & Printing Med Rec Reports Medication Reconciliation Reports are found under the “Pharmacy” tab for nursing associates and under the “Nursing” tab for pharmacy associates. This should be the same as the previous system. All electronic Medication Reconciliation Reports are located under this item.

Accessing & Printing Med Rec Reports All electronic Medication Reconciliation Reports are located under this item. Option #1 – Allows the nurse to print the electronic home med list, which was entered by the pharmacist from the “orange sheet”. This is not a reconciliation report.

Accessing & Printing Med Rec Reports Option #2 – Allows the nurse to view the electronic home med list that has been entered by the pharmacist.

Accessing: Transfer Med Rec Report Option #3 – Allows the nurse to print the “Transfer Medication Reconciliation” report. This report will allow the MD to continue meds during transfers.

Accessing: Discharge Med Rec Report Option #4 – Allows the nurse to print the “Discharge Medication Reconciliation” report. This report will allow the MD to document meds to be taken at home.

Accessing: Discharge Med Education Option #5 – Allows the nurse to print the “Discharge Medication Education” report. This report will assist the RN to educate patient about meds to be taken at home.
