1 New SFGH electronic discharge process: Medication Reconciliation E-Prescribing Electronic PDP (Provider Discharge Plan) and summary Electronic Discharge Instructions

2 Why go from paper to electronic? Enhance consistency of discharge process & efficiency (using LEAN principles) Improve transitions of care, increase data portability (HIE) Comply with federal mandate from CMS (ACA and JC) Improve patient experience (legibility, comprehensiveness of documentation)

3 When/Where: April 29 th house-wide rollout All med surg units affected (4B, 4D, 5A, 5C, 5D, 6A adult patients, 7D) Home med collection begins in ED ePDP/Discharge Instructions print on med/surg units ePrescriptions delivered to Discharge Pharmacy and community Pharmacies Providers complete all aspects of discharge process electronically, independent of location
