MD Before DT MD During DT MD after DT Enter transfer orders for potential transfers Reference  StarPanel  and paper copy of orders in the Medical Record (pt. chart) at the patient bedside/nurses station DO NOT ENTER ANY ORDERS WRITTEN DURING DOWNTIME- IT MAY RESULT IN DUPLICATES Enter TPN orders early to be able to use VGR and to assure TPN is made in time TPN ordering- VUH New orders- write Order to consult TPN team, TPN Pharmacist will complete order form Existing TPN- TPN team will adjust TPN Sun am based on labs. VCH- complete TPN Ordersheet (in E-DOCS) No requirement Enter Discharge orders for potential discharges including completing discharge letter Write Orders on Physician Order Sheet (POS) in the Medical Record (Pt. chart) Sign orders, print name, and denote contact #(beeper, etc) Use only Black  Ball Point Pens One order sheet per set of orders, X out blank space Use approved abbreviations Order sets will be available online for reference SSS website with other DT resources: Pharmacy will enter medications, Nursing will enter non medication on going orders. It may take several hours for all orders to be “caught up” Renew restraint and pain orders Admissions Orders- use POS and be sure to include Attending, Service, Team and allergies Transfers Orders ONLY- denote  orders on the Transfer Order sheet in the pt. chart- line through orders to discontinue, denote edits, write new orders on blank lines Discharges Orders- write discharge orders and discharge instructions on POS.  Enter Rx in RxStar as usual Special Considerations Consults Write order for ”consult (service)” and then provider to contact consulting provider (call operator for # or reference list on units). Insulin Drip & Electrolyte Replacement Protocols- RNs will adjust rate according to protocol Nurse Managed Heparin Protocol see Protocol in E-Docs or in SSS website Code Status orders- Enter in StarForm and ALERT THE RN.
