SESSION OBJECTIVES 1. Describe key components of team based care in the primary care setting. 2. Understand the key role of Behavioral Health in the Primary Care team. 3. Describe examples of creative solutions to increase access to Behavioral Health in Primary Care. Session Structure and Questions Introductions of panelists – name and role in primary care Kathy to go through PowerPoint slide Questions and answer session Provide some insights about why team based care works well in PC How do patients and families benefit from receiving their care from interdisciplinary care teams? How can primary care clinics best use behavioral health? Who are the different professionals that can be used on a BH team? What are some of the barriers to accessing BH in primary care? Discuss some of the creative solutions you’ve implemented when assisting patients in accessing BH How important is collaboration other clinics, community partners, hospital system partners?
