Facility Requests Facility/Game Request Form Fill this out to request to host an event/game/request facility for meetings (even if during practice time) Should be turned in 4 weeks in advance for weekend games/events Should be turned in 2 weeks in advance for game/event during practice time/meeting space Practice Request Forms due Friday of Week 3 for following term 1-2 practices per week on campus; 2 hour time blocks

Contracts, Fundraisers & Sponsorships Students and coaches ARE NOT authorized to sign contracts, including game contracts. Any forms needing to be signed by the Registrar’s Office must be submitted at least 2 weeks prior to due date. All Club Sports Fundraisers must be submitted for approval at least 5 business days before the event is to take place. Clubs who would like to enter into a sponsorship agreement with any company/organization must request approval through Club Sports Staff BEFORE any type of agreement can be made, money can exchange hands, or logos may be placed.
