4. GAME AND EVENT FACILITY REQUESTS All clubs must fill out a Game and Event Facility Request to be able to schedule a one time practice or event. Must be turned in 2 weeks prior to event.

FINAL DAY TO SUBMIT PURCHASE REQUESTS AND INTENT TO TRAVEL FORMS USING FY 14 RESTRICTED FEE ALLOCATIONS 5/2/14 If you have more than 5% of your original restricted fee allocation at the beginning of the year still in your account after May 3 rd, your club will be penalized 50% of what is left in next years allocation. Ex. Curling Club had $480 (8% of their budget) left from their FY14 allocation of $6000. They will be penalized $240 of the FY15 allocation of $5000. Therefore, they will be left with $4760 for FY15.

8. RENEWAL OF RECOGNITION Due 4/23/14 *** Do this in advance – must have advisor’s signature!

9. EQUIPMENT CHECK-IN All club equipment owned by the Rec must be checked-in between the dates of: 4/28 – 5/9 You must set up a time to check it in with your program manager.
