The Current issue to be addressed Limited allocation of time to each patient with only certain staff to provide oral/dental care. Nutrition can be affected by the following oral health related problems: Pain and discomfort when chewing (due to no teeth- “endeulous”) Swallowing problems that can lead to malnutrition Infectious diseases of the mouth- such as periodontal disease and caries. Immune system problems- due to poor oral health that leads to aspiration. (S.O. Griffin, Jones, Brunson, P.M. Griffin, Bailey, 2012)
Interdisciplinary approach to oral health If all staff come together, it can ensure that oral health care is provided through the use of knowledge from each profession. (Yellowitz, 2016) Social worker Nurse Doctor *Ensure best care for patient. *Arrange access to dental care after discharge. *Note changes. *Education about prevention. *Cleaning *Note changes *Comprehensive emergency care. *Cleaning *Note changes Psychologist *Counselling related to causes of dental problems *Note changes in behaviour/ distress *Review if there is an oral sensory process dysfunction. *Prescribe assistive technology. *Cleaning *Note changes. Pharmacist *Review medications *Education in regards to opening the mouth and chewing. (Focus on jaw function). * Note changes. Occupational Therapist Physical Therapist
Processes of regular cleaning Effective tooth brushing for at least 2 minutes Use either a manual or powered toothbrush If a manual toothbrush is used ensure it has a small head with medium texture bristles Brush dentures 2 to 3 times a day; remove at night and soak in clean water Brush teeth and gums after each meal with a fluoridated toothpaste (Daly & Smith, 2015)
Routine comprehensive oral health screens (Livewell Southwest Healthcare, 2016)
Intended positive outcomes Better nutritional health. Reduce pain and discomfort when swallowing. Holistic care provided by all staff. Regular comprehensive checks to prevent major problems. Better overall health. Staff working together to reduce time burdens and to report findings.
Conclusion Oral health care is everyone’s priority! We need to work together to provide effective, timely and comprehensive oral health care for the older people with dementia in our care.
