Community Learning Disability Teams Teams made up of RNLD Nurses Therapists e.g. Specialist OT, Speech and Language Physiotherapists Consultant Psychiatrists Can provide: Health facilitation Health promotion Advice and training Support around behaviours deemed as challenging

Health Facilitation Role This involves a focus on individual health outcomes and is recognised as a key role that could be taken on by a range of people including support workers, family carers, friends and advocates as well as health professionals.

Example Community LD team can offer support by; Jo is a 26 year old person with moderate learning disabilities and autism. It has been noticed that when accessing social activities Jo has been prone to falling / tripping. Family report that Jo has never had her eyes tested. Jo gets frightened in unfamiliar places. Community LD team can offer support by; Offering desensitisation Developing a health action plan outlining reasonable adjustments. Working together with opticians to understand needs and build relationships.

Do they have complex physical / mental health needs or behaviour that is presenting as a barrier to them receiving care or treatment from a health care service? Has the person got a diagnosed learning disability? If yes do you need advice or support from the Community Learning Disability Team?
