2 Who are we? We’re from the Transforming Care Partnership in south West London Jill Mulelly – patient and public engagement Elaine Woodward, Senior Strategy Manager

3 Why are we here? We want: To talk to you about what is happening with the Transforming Care Partnership in your local community Get your advice on how to speak to people with learning disabilities about this work

4 What is a Transforming Care Partnership?
The Transforming Care Partnership is a group of professionals looking to improve services for people with learning disabilities. There is one in each area of London. We have been asked to help take forward a new plan called ‘Building the right support’. The plan explains how we can improve services for people with learning disabilities and or autism.

5 Why was this set up? Some people with learning disabilities still live in places called assessment and treatment units. Assessment and treatment units are like special hospitals They are different to the hospitals that you go to when you get hurt or sick. People sometimes stay in these special hospitals when their behaviour is difficult for others to manage.

6 The plan says, people with learning disabilities must be supported to:
Live where they choose Get support with their health in the community, close to where they live. Get health treatment near their home when they need it Only stay in hospital for as long as they need to.

7 We know… 51 people from south west London are in special hospitals. These hospitals are not in south west London 6 of these people are under the age of 18

8 Paying for the changes The government wants us to spend money differently to get better services Spending money on community services instead of hospital care. Using the money they save when they reduce the number of special hospitals.

9 People must be at the centre of our plans
We need to work together with people who have Learning Disabilities and/or autism so that services are better in the future

10 How we are working together
Wider reference group – we have formed a group of local organisations and carers to tell us their experiences We would also like to talk to people who use services – and we need your help.
