ACTION PLAN INTRODUCTION ? What is an action plan and why is it used? An action plan is a document that clearly describes the steps needed to reach a specific goal ? Can we create an action plan for QI? Yes! An action plan can be a helpful tool to outline the activities and resources required to meet any goal (including QI!) and assign specific roles to stakeholders to increase accountability Final QI Workshop

ACTION PLAN COMPONENTS Action Step or Commitment By Whom What needs to be done? What partner or type of RN member will do the action? By when or Frequency Resources or Support Needed Resources or Support Available What additional resources or support are needed to carry out the action? By what date or how frequently will the action be done? What resources or support are already available (financial, human or other)? Communication Plan Potential Barriers or Resistance Who should be informed about/be involved with this action? What issues might prevent this action from occurring? Final QI Workshop

HOW CAN AN ACTION PLAN BENEFIT OUR QI WORK? An action plan is valuable because it: Is a statement of commitment by the referral network to carry the QI work forward Clearly details how QI will be continued, including what actions will be taken, by whom and what resources are needed to do that Increases accountability of the stakeholders included in a plan to carry out their commitments Final QI Workshop

ACTION PLAN FOR QI ? How can the referral network use an action plan for QI? An action plan for QI will document: The QI-related activities the referral network will commit to carrying out beyond the initial QI program The resources needed to ensure those activities occur A timeline for when the activities will take place Final QI Workshop

BRAINSTORM QI COMMITMENTS Session Objective Work individually or with a partner to brainstorm ideas for QI commitments to be included in the QI action plan Final QI Workshop

WHAT IS A QI COMMITMENT? A QI commitment is an activity an individual referral network member, or their organization, pledges to doing to assist the referral network to meet its QI goals informed by the QI priority areas Example: ‘As the QI team leader, I will continue to organize QI meetings every month’ ? Who can think of another example of a QI commitment? Final QI Workshop

BRAINSTORMING QI COMMITMENTS We are now going to think individually, or in pairs, about the commitments we are willing to make to QI, keeping in mind the referral network priorities discussed previously and what will be needed to make progress in those priority areas. Take 15 minutes to develop 3-4 QI commitments that you will contribute during the QI action plan development process ACTIVITY Final QI Workshop
