Presentation transcript:

Do you have a long term condition such as chronic pain, diabetes, chronic fatigue/ME or cardiovascular problems? Cognitive Behaviour Therapy (CBT) might help you manage your symptoms and improve your mood! Long-term conditions are defined on the Department of Health website as “those conditions that cannot, at present, be cured, but can be controlled by medication and other therapies.” More than 4 million people in England with a long-term physical health condition also have mental health problems, and many of them experience significantly poorer health outcomes and reduced quality of life as a result.

Kirklees and Calderdale IAPT (Improving Access to Psychological Therapies) is part of the South West Yorkshire Partnership NHS Foundation Trust and has been providing NICE guidelines compliant, evidenced based therapies to people in the Kirklees area for the past 8 years. Kirklees and Calderdale IAPT work with long term conditions to help people lead more active and fulfilled lives and currently run the following groups throughout Kirklees: Living well with a long-term condition Living well with pain Living well with diabetes Living well with pain and fatigue All our groups are based on evidenced based Cognitive Behavioural Therapy Techniques and are run by qualified and experienced Cognitive Behavioural Psychotherapists and Psychological Wellbeing Practitioners.

In addition we offer individualised Cognitive Behavioural Psychotherapy to help manage the depression and anxiety that often come with long term conditions and to help people get the most out of life again. Ask your doctor about how our service could help you or call our friendly admin team on 01484 343700 www.askforiapt.co.uk
