The Centers For Disease Control and Prevention The Centers for Disease Control and Prevention (CDC) serves as the national organization for the development and application of disease prevention and control, environmental health, and health promotion and health education activities to improve the health of the people of the United States. There are several Centers . National Center for Chronic disease Prevention and Health Promotion. (NCCDPHP)

The Division of Diabetes Translation (DDT) Established in 1987 as the lead agency in the government for the translation of diabetes related activities, Division in the National Center for Chronic Disease Prevention and Health Promotion. Facilitates the widespread application of diabetes research into clinical and public health practice

Prevalence of Diabetes in the United States Why the increase in spite of efforts? Increase in prevalence of obesity and other diabetes related risk factors driving up the number of new cases (incidence) Reductions in mortality rates People are living longer People are living longer with diabetes due to inceased access to health care and better quality care.

Barker et al: In 2010 an article was written in the AJPM classifying most of Appalachia as in the diabetes belt. In other words their prevalence of dm was 11.7 % ad compared to 8-9% in the rest of the US at that time, Now based on our county level data in 2013, the prevalence of Dm in the distressed counties ranges from 11.7 % to 17%. The range in the prevalence of obesity in the distressed counties is 29.1-41.4

The ADCTP Project IAA with ARC since 2000 Focused on the distressed counties in Appalachia. Center for Rural Health, Marshall University – a strategic local partner As of March of 2016 there were 93 distressed counties in Appalachia.

The Current Status of ADCTP 73 coalitions and 60 still active in 2015 (80%)

Prevalence of diabetes in the United States

National Diabetes Prevention Program This is a public-private partnership of community organizations, private insurers, health care organizations, employers, and government agencies. Partners work to establish local evidence-based lifestyle change programs for people at high risk for type 2 diabetes.

The National Diabetes Prevention Program Science for prevention of Diabetes( 5-7% of body weight and 150 min of PA) Implemented pilots and demonstration projects with the YMCA and a variety of insurance companies. State and Local Programs

ADCTP and the National Diabetes Prevention Project ADCTP Coalitions have been involved in the development of the National Diabetes Prevention Program Coalition members supported pending and recognized programs Referred 132 participants to National Diabetes Prevention Sites in 2014 New IAA for 2016 will re-focus efforts in the distressed counties on the expansion of National DPP in the Region. 72 completed the course and the others are on the way to completion.

2017 Performance Measures Recruit and support participants to Pending and Recognized National DPP in distressed counties in Appalachia. Number of persons with prediabetes or at high risk for type 2 diabetes who enroll in a CDC-recognized diabetes prevention program. Number of people with diabetes who have at least one encounter at a DSME program during the funding year (emphasis on ADA-recognized or AADE-accredited DSME programs that meet national quality standards). The focus next year on new reporting will be for the first two measures.
