2 National Kidney Foundation of Michigan Prevention is number 1 priority Nearly 90 cents of every dollar raised goes to programs. Charity Navigator rates the NKFM as one of America’s best charities. Huffington Post named it as 1 of 11 charities that changed the world in 2014.

3 What NKFM Can Offer Diabetes Prevention Program Enhance Fitness A variety of other evidence-based programming Worksite Wellness

4 Diabetes Prevalence in Michigan In 2014, an estimated 10.4% of Michigan adults 18 years and older were diagnosed with diabetes — 799,350 people According to the Centers for Disease Control and Prevention (CDC), 27.8% of people of all ages with diabetes are undiagnosed. 37% of adults age 20 years and older were estimated to have prediabetes, putting them at high risk for developing type 2 diabetes. However, in 2014, only an estimated 8.2% of Michigan adults reported ever being told that they had prediabetes.

5 Diabetes Prevention Program and NKFM Called My Choice... My Health Diabetes Prevention Program- a program of the National Kidney Foundation of Michigan. Helps people at risk of developing type 2 diabetes make achievable and realistic lifestyle changes to cut their risk of developing the chronic disease. My Choice... My Health is a year-long program with 16 weekly sessions and 6 monthly follow-up sessions with trained lifestyle coaches who empower participants to take charge of their health.

6 NKFM and MCMH-DPP Deliver the program in a variety of settings: clinics, faith-based organizations, worksites, and community organizations More than 700 participants since launching the program in 2012 achieving an average overall 5.3% weight loss First DPP program to be fully recognized by the CDC 92% of participants are “very satisfied” with their NKFM program 67 trained lifestyle coaches across the state

11 Worksite Wellness MDHHS Getting to the Heart of the Matter

12 Worksite Wellness Getting to the Heart of the Matter in Michigan -- a collaborative project at the Michigan Department of Health and Human Services. Goals of the project: Increase healthy behaviors Decrease obesity, and decrease tobacco use Provide resources Join partners in your community to meet these goals

13 Worksite Wellness Worksite Criteria: Interested in increasing opportunities for health and wellness at their work place Through policies, environmental changes and programs Have 50 or more employees Implement quickly

14 On average, Americans working full-time spend more than one-third of their day, five days per week at the workplace Employers have abundant opportunities to promote individual health and foster a healthy work environment Effective workplace programs and policies can reduce health risks and improve the quality of life for workers Why should we promote a healthy workplace?

15 Promoting a Healthy Workplace Step 1. Establish a worksite wellness committee (or appoint an individual) that will champion the efforts and establish the kinds of initiatives you’d like to focus on. Topics can include: healthy eating physical activity tobacco cessation mental health

16 Promoting a Healthy Workplace Step 2. Take the DHEW (Designing Health Environments at Work) – A workplace environment assessment Step 3. Create an Action Plan The DHEW site and NKFM will help you with this. Your Action Plan will include short term and long term activities that will help you meet your organization‘s wellness goals.

17 Promoting a Healthy Workplace Step 4. Establish policies to promote a healthy environment. Some examples include: Offering nutritious food and beverage options at your meetings Support healthier choices, provide leadership and role modeling, and help to create a social norm around healthier choices and behaviors. Implement a walking meetings, time for physical activity
