Family Planning Program Administration Records are complete & have appropriate documentation (Internal Medical Audit) Direct Patient Care & Patient Education Customer satisfaction: All patients questions are answered, patients served in a timely manner. Metrics: Internal Medical Audit (report favorable) Metrics: Customer survey results # total patients. Family Planning Program To provide affordable, confidential, quality reproductive health services. Fiscal Management Family Planning is a fiscally sound and sustainable program. Community Education & Outreach Community is aware of Family Planning services. Community is educated on reproductive health and STI’s. Condoms are accessible to community members. Metrics: Family Planning is operating within budget (refer to monthly FAS) Metrics: # of Native American clients. # of condoms distributed. # of outreach events. Billing & Eligibility Billing is completed timely & efficiently. Patients are screened for financial eligibility so that the right people get the right program. Professional Development Family Planning staff will follow the Workforce Development Plan. Metrics: % of clients with a financial screen annually. # of claims past 120 days and/or denied claims. Chart review (bi-annual) Metrics: See Workforce Development Plan

Hill County Public Health Family Planning Billing & Eligibility Home Visiting Education & Outreach Immunization Hill County Public Health Administration of Immunizations WIC Quality Assurance Based on ACIP Recommendations Communicable Disease Professional Development Emergency Preparedness

Hill County residents are protected from vaccine preventable diseases. Billing & Eligibility Clients are screened and linked to the appropriate immunization programs. Billing is completed timely and efficiently. Quality Assurance Based on ACIP Recommendations Vaccine Information Sheets are current & distributed at each visit Vaccine integrity is maintained Best practices from the CDC and ACIP are followed to ensure: client safety; staff safety and education; and 6 R’s. Standing orders are current and reflect ACIP recommendations Metrics: # of program assignment inaccuracies. # of claims past 120 days. Education & Outreach In Hill County, there is a provider coalition focused on immunizations. (HIT) Daycare immunization records are screened for up-to-date status. Provide 3-4 community educational opportunities about immunizations. Work with schools to ensure compliance with immunization rules. Metrics: # of cold-chain temperature excursions Standing orders reviewed annually Immunization Program Hill County residents are protected from vaccine preventable diseases. Metrics: # of meetings. (HIT) % of licensed daycare reviews. # of community educational opportunities given about immunizations. Professional Development Immunization program staff will follow the Workforce Development Plan. Metrics: See Workforce Development Plan Administration of Immunizations Patients are educated on CDC and ACIP recommendations at every encounter to ensure there are no missed opportunities. Timely completion of inventory. All consented immunizations entered into the state registry (end of each month). Metrics: # of immunization errors (related to 6 R’s). Vaccine inventory matches immunizations given (monthly). # of missed opportunities (DPHHS report).

Nurse-Family Partnership/ Home Visiting Community Outreach NFP will be promoted within the community. Metrics: Total # of referrals # of NFP outreach activities Nurse-Family Partnership/ Home Visiting Promote health and development for first-time, low-income pregnant women and their infants by: Improving school readiness Decreasing abuse and neglect Increasing self-sufficiency Increasing appropriate referrals Increasing positive pregnancy outcomes Program Operations NFP grant and program requirements are met. NFP model fidelity is met. Public Health Concepts are promoted. Metrics: # Active clients. # Group Connections offered. % of children enrolled in NFP who are up-to-date on immunizations. Professional Development NFP staff will follow the Workforce Development Plan. Metrics: See Workforce Development Plan

Professional Development Data Collection Collect accurate data that can provide a clear picture of the health needs of WIC clients. Staff maintains calibration equipment. Administration Perform all aspects of client visit. Ensure retail stores are compliant with WIC guidelines and rules. Customer Satisfaction: All patients questions are answered, patients served in a timely manner. Metrics: Based on category, # of women & children with low iron. Based on risk code, # of referrals to Registered Dietician. # of months between equipment calibration. Metrics: Score on the DPHHS monitoring site visit (2 years) # of compliant retail stores (3 years or as needed) # of cashed vouchers Future: Customer survey results Referrals & Outreach Provide and assist with referrals to community resources. Educate community of WIC program & benefits. WIC Program Increasing opportunities to access healthy foods, improving nutritional understanding, nutritional health, and breastfeeding support for Women, Infants and Children in Hill, Liberty, & Blaine Counties. Metrics: # of appropriate referrals. # of outreach activities per year Education/Support Provide nutritional/breastfeeding education through evidence-based resources. Provide support and guidance to families that allows them to make informed decisions. Metrics: Breastfeeding rates (3, 6 months) # of Nutrition Education visits (score on the DPHHS site visit) Fiscal Management WIC is a fiscally sound and sustainable program Metrics: # of months WIC is operating within budget Average # of participants Professional Development WIC staff will follow the Workforce Development Plan. Metrics: Ensure timeliness and completeness standards are met Reconcile QI-4 reconciliations in one year Communicable Disease Prevent and control the spread of Communicable Disease through investigation,education and treatment in Hill County’s jurisdiction. Community Education Community is provided information on communicable disease issues. Metrics: # of media outlets to provide increased community awareness. Professional Development Communicable disease staff will follow the Workforce Development Plan. Metrics: See Workforce Development Plan

Emergency Preparedness Support preparedness for and response to emergencies, threats, and disasters with Public Health implications. Metrics: % of HANS distributed & forwarded to the State % of responses to tests of HAN system % of successful 24/7 notification system (State) % of successful 24/7 notification system (Local) Emergency Preparedness Upgrade and enhance local Public Health capacity to respond to events impacting public health, through planning, assessment, and development of preparedness and response activities.
