3. WithinReach makes the connections that all Washington families need to be healthy.
For the last 25 years WithinReach has connected vulnerable families to health care and food resources through innovative technology-based and in-person outreach. In 2012, we connected 220,000 people across Washington State to resources— including Medicaid, CHIP, Basic Food, and WIC— creating healthy families and healthy communities.
By meeting people’s basic food and health care needs, WithinReach can intervene early and often prevent a larger more costly health or financial crisis. Preventing a crisis and making a family more resilient helps to support the overall long-term health and wellness of a family and the community.
WithinReach extends the impact of direct service to individuals and families by working in partnership with community organizations and state and local agencies. WithinReach’s direct service and coalition mobilization work are focused on five core areas which creates the best opportunity to promote healthy families and healthy communities.

4. BREASTFEEDING IMMUNIZATION POLICY
Provide a state-level support system to improve care for breastfeeding moms and babies.
Promote immunizations towards an 80% goal as an important way to keep kids and families healthy.
Ensure that laws, policies and programs work to support the health of children, families, and
our communities.
Breastfeeding: Only 20% of babies in Washington are breastfed exclusively for the recommended first six months of life.
Immunizations: Washington state has the seventh highest vaccine exemption rate in the country.
Policy: When the law in Washington works on behalf of families and children, the future is more secure.

5. CHILD DEVELOPMENT HEALTH CARE FOOD ACCESS
Provide developmental screenings free of cost so that families can monitor their child’s physical and social-emotional development.
Connect people access the state with the health care resources they need, to promote long-term health and wellness.
Connect people across the state with food resources that help them access better nutrition.
Child Development: Nearly one in six Washington kids has a developmental delay. Children with untreated delays are more likely to experience low academic attainment, low lifetime earnings, and poor health.
Health Care: More than half a million people will qualify for Medicaid under the Affordable Care Act guidelines.
Food Access: Washington is now the 23rd hungriest state in the country.

16. Client Information
As subcontractors with DSHS it is very important to maintain client records confidentially.
Keep paper records (names, DOB,SSN) filed away and locked when not in use.
Electronic files should be stored on a secured server where only authorized personnel have access.

20. CSD Non-Disclosure Form
CSD Non Disclosure Form must be signed annually for all staff who assist with applications or have access to a client’s DSHS information.
