Background & History: National Began in 1982 in Peoria Promise: To end breast cancer forever Largest grassroots network of survivors and advocates $2.2 billion invested so far

Background & History: Locally Serves 91 counties in Iowa Began Race for the Cure in 1991 Invested more than $4 million in last 5 years 25% of proceeds support national research efforts New efforts to formalize grant process Broad call for grant reviewers RFA focuses on needs from the Community Profile Improved communication: grantees and staff/board

Community Grants Overview Funds projects in areas of: Breast health education Improving access to the continuum of care Improving the quality of life of breast cancer patients and survivors Priority given to needs identified in the Community Profile

Eligibility Located in or serve 91-county area Specific to breast health or breast cancer Tax exempt under Internal Revenue Service In compliance with all Komen requirements

Changes for Priority given to needs identified in Community Profile Increase knowledge and awareness of mammography screening services, with a specific focus on decreasing the percent of women between 40 and 70 years of age who have not had a mammogram in the last two years. Increase the ability to access services in an effort to improve the quality of life throughout the continuum of care Increase knowledge in evidence based-breast cancer screening promotion practices among health professionals in each Target Area, with emphasis on all “at risk” populations.

Review Process Independent and anonymous panel of community members Impact: 30 points Feasibility: 25 points Capacity: 15 points Collaboration: 10 points Sustainability: 10 points Priority: 10 points Final slate presented to the board for approval Awards made in March for April 1 start date

Grant Management Contract to each grantee in March Half of award sent with completion of contract Interim report due October 1 Second check sent after interim report accepted Final report due May 1 Site visits Need your stories of impact of Komen funding

Small Grants Program In revision, plan to start accepting applications in January 2014 Address goals of Susan G. Komen Funds set aside in the budget No bonus for addressing priority areas An organization may only be awarded one per year
