Rural Health Network Planning 101 Purpose - To assist in the planning and development of an integrated health care network. Goals: 1.Focus on the value of services and not volume. 2.Create a foundation for infrastructure. 3.Create the opportunity for collaboration between different members of health care delivery system. 4.Address emerging community health needs and challenges.
Rural Health Network Planning 101 Estimated Number of Awards:24 Maximum Award Amount: $100,000 Project Period: June 1, 2016 – May 31, 2017
Eligibility Requirements The lead applicant must meet 3 eligibility elements – 1.Rurality, 2.Non-profit/public designation, and 3.Have a network composed of at least 3 separately- owned organizations.
