4 Purpose: To provide support to rural primary care providers for planning and implementation of quality improvement activities. 5 Goal: Promote development of an evidence based culture and delivery of coordinated care in the primary care setting Objectives: Improved health outcomes Enhanced chronic disease management Better engagement of patients 5 Goals and Objectives

6 Overall Outcome Demonstrate improvement in health status and to show a reduction in emergency department visits due to chronic disease

7 Identify patients with highest health care utilization due to chronic disease Align with Delivery System Reform (DSR)  Care delivery – use quality improvement model  Information sharing – use health information technology (HIT)  Payment incentives – incorporate value-based payment incentives Collect and report data Project Requirements 7

10 Funds under this announcement may not be used for the following purposes: To build or acquire real property; or For construction or major renovation. NOTE: The purpose of this grant is to fund quality improvement activities that will lead to a reduction in emergency department visits due to chronic disease and improved health status.
