4 Program Overview Act 10 of 2013 established the Community-based Health Care Program within the Department of Health The purpose of the Program is to increase access and/or improve services to the underserved, uninsured or underinsured population

5 Program Overview Category 1: Development of a new community-based health care clinic Category 2: Expansion of primary health care services at an existing community-based health care clinic Category 3: Addition or expansion of prenatal, obstetric, postpartum, and newborn care services at an existing community-based health care clinic

6 Program Overview Category 4: Development of alternative health care delivery systems at an existing community-based health care clinic to improve services and access to reduce emergency room utilization Category 5: Implementation of collaborative relationships to enhance transition of care for clinic patients seen in or admitted to the hospital and to develop referral mechanisms to establish a medical home for those patients seen in the hospital or specialty clinics

7 Program Overview Must be a community-based health care clinic that provides comprehensive primary health care services Must be one of the following types of organizations: FQHC, Certified RHC, hospital health clinic, free clinic or nurse-managed health care clinic

8 Program Overview The proposed clinic site must be in a federally designated HPSA (health professional shortage area), MUA/P (medically underserved area or population) or an RHC (certified rural health clinic) OR

9 Program Overview If the clinic site does not have one of those federal designations, the organization must demonstrate it has served at least 30% low income patients

11 Program History RFA 67-26 was posted in July of 2013
66 applications received 40 grant awards $7.9M funded over the 18 month period January 2014 to June 30, 2015 (13-14 and state fiscal years)

12 Program History RFA 67-28 was posted in March of 2014 55 applications
17 grant awards $3.5M funded for the period Oct. 1,2014 to June 30, 2016 (14-15 and state fiscal years)

13 Program History RFA 67-34 was posted in March of 2015 42 applications
26 grant awards (Category award amounts were increased) $7.7M funded for the period Oct. 1, 2015 to June 30, 2017 (15-16 and state fiscal years)

14 Program History RFA 67-46 was posted in November of 2015
30 applications 12 grant awards $4M funded for the period July 1, 2016 to June 30, 2018 (16-17 and state fiscal years)

15 Program History State Fiscal Year 16-17 budget was reduced to $2.1M
Required to do a funding reduction for Grantees We had no funding left to post an RFA for grants beginning July 1, 2017.

16 Program History RFA 67-64 was posted in September of 2017
32 applications 11 grant awards $2.1M funded for the period July 1, 2018 to June 30, 2020 (17-19 and state fiscal years)

17 Program Future Act 10 requires this program to be funded each fiscal year With $2.1M, the Department will post an RFA every 2 years Funding at this level makes this procurement very competitive

Increase access to care for the uninsured, underinsured and the underserved populations Increase the number or primary health care professionals in your area

19 Grant Writing Tips Read the RFA very carefully
Choose the appropriate category for your proposed project Develop your proposed project plan with specific measurable outcomes, goals and activities (work plan) Respond/answer/address everything asked for in the RFA

20 Grant Writing Tips Follow instructions for completing the budget and budget justification Don’t submit an application from previous RFAs Don’t provide additional information not requested in the RFA Proofread the submission carefully Have someone else proofread
