South Tyneside Lifestyles Physical Activity levels still poor –More than 60% do not engage in even moderate exercise 52% in overweight/obese category 30% of “obese” (13% increase since 1992)

Aims and Objectives To address health inequalities in South Tyneside by encouraging individuals to join the programme. To assist residents of East Shields to choose a healthier lifestyle. To assist with the Horsley Hill Neighbourhood Action Plan – Improving Health theme

The Facts…. Being overweight can lead to: –Heart disease –Diabetes –High blood pressure –High cholesterol –Bone and joint problems –Difficulty with day to day tasks –Anxiety and/or depression –Breathing difficulties –Tiredness –Isolation

Losing weight….. To lose weight, our bodies need to take in less energy from food, whilst using more energy through physical activity. It may sound simple but takes planning and determination………. That’s why we are here to help!

The Choosing Health Team The Choosing Health Team is made up of 6 Health and Lifestyle Advisors and 6 Community Health Officers. Your Health and Lifestyle advisor will: –Design individually tailored 12 week programmes to each clients needs, that aim to: Increase opportunities to access physical activity Improve diet –Both of these help weight management Stop-smoking signposting Improve physical and emotional well-being Give support and encouragement

Your Community Health Officer will… Address and meet the health needs of the clients in the community Establish and maintain groups and activities to help clients establish a routine To support, advise and motivate client’s over the 12 week period through ‘Health Clinics’ and after the 12 week period through ‘Drop in’ sessions.

Community Bases in East Shields Appointments can take place at: –Horsley Hill Community House 1 Amble Avenue, Horsley Hill –Ridgeway Family Centre Park Avenue, Cleadon Park –Cleadon Park Library Sunderland Road, Harton

Current Activities –Weight management course (accredited) –Health Clinic and drop in service –Gym sessions (separate men's and ladies) –Badminton session –Tai Chi –Swimming –Aquafit –Further activities across other CAF areas (e.g. walks, cycling, aerobics) –Oh, and yes, they are all FREE!

How to join the programme There are 2 ways to access the programme 1.Referred by Health Professional (e.g. nurse, health visitor, GP etc) 2.Self referral.
