Section 504 Pasco County Procedures

Beginning of the Year 504 Procedures Make sure that every student with an existing 504 Plan has their own 504 folder that is in the cumulative folder room All documentation on the 504 Checklist should be included in the 504 folder Make sure all students with an existing 504 Plan are entered into PlanMaker/504

Beginning of the Year 504 Procedures (continued) Disseminate a copy of existing 504 Accommodation Plans to ALL teachers that work with each 504 eligible student, including Specials/Electives teachers. Document receipt/review of 504 Plans (Receipt for Existing 504 Accommodation Plan Form) Should occur anytime student has new teacher(s) during the school year Towards the end of the first quarter (October), all teachers must sign Annual Accommodation Plan Review Form and return it to the 504 Coordinator

Beginning of the Year 504 Procedures ALL teachers must sign the Annual Accommodation Plan Review form and return it to the 504 coordinator. After review, teacher(s) meet with 504 Coordinator to determine whether to continue or revise current plan  If 504 Plan is to be continued without change:  Mark “Continuation” on plan in PlanMaker/504  Update teacher name and date on 504 Accommodation Plan in PlanMaker/504  Print-out updated 504 Accommodation Plan in PlanMaker/504 for teachers & admin to sign  Original signed copy (with new teacher signatures) goes into cum/504 folders  Distribute photocopies with new teacher signatures to all teachers  Send to parents: photocopy of plan with new signatures, copy of Parental Rights, Parent Notice of Section 504 Committee Meeting, and McKay option letter  Complete new Data Entry Form & submit form to data entry clerk

Procedures Continued… If 504 Plan is to be REVISED: ◦ Schedule 504 committee meeting ◦ Send home Notice of Evaluation/Reevaluation, Procedural Safeguards, 504 Consent, and Parent Input Form ◦ Collect new data on student to support suggested modifications to 504 Accommodation Plan ◦ Hold 504 meeting and review relevant data  Complete Section 504 Committee Report Cases Under Review or Re-Evaluation  504 team considers changes / applicable accommodations  Enter revised plan into PlanMaker/504 – Mark “Revised”  Update teacher names in PlanMaker/504, get signatures  Original signed copy (with new teacher signatures) goes into cum/504 folders  Distribute photocopies with new teacher signatures to all teachers  Send to parents: photocopy of plan with new signatures, copy of Parental Rights, Parent Notice of Section 504 Committee Meeting, and McKay option letter  Complete new Data Entry Form & submit form to data entry clerk

Initial Plans Parent contacted by school-based personnel regarding concern Parent is sent home a Parent Consent, Parent Rights, and Parent Input Form ◦ WRITTEN PARENT CONSENT IS REQUIRED Collect data to determine the need for a 504 Schedule a 504 Eligibility Determination Meeting following data collection

Initial Plans Parent is sent home Notice of Section 504 Evaluation/Re-evaluation form Conduct 504 Eligibility Determination Meeting and decide if there is: ◦ Evidence of impairment? ◦ Evidence of substantial limitation? ◦ Evidence of need for accommodation(s)? Complete Eligibility Determination Committee Report

Initial Plan (continued) If 504 plan is agreed upon: Create 504 plan on PlanMaker/504 (Mark as “Initial”) ◦ Get required signatures and provide copies Send home to parents: ◦ Parent Notice of Section 504 Committee Meeting form ◦ Copy of 504 plan with signatures ◦ Copy of McKay option letter If 504 Plan is created, have data entry clerk enter date into TERMS (504 DATA ENTRY FORM)

Common Errors in Development of 504 Plan Failing to match services and accommodations with student needs Failing to provide copies to all responsible for implementation Writing vague plans Insufficient monitoring School administration not involved with monitoring Plan does not address non-academic and extra- curricular activities, if appropriate 12
