1 PSRC EXCEPTIONAL CHILDREN Compliance issues and suggestions that will help with case management

2 KNOW THE STATE FORMS  DEC-1 - E. C. Referral  DEC-2 – Permission to Test  DEC-3 – Testing Results  DEC-4 – IEP  DEC-5 – Parent’s Prior Notice  DEC-6 – Permission to place  DEC-7 – Re-evaluation Determination  Invitation to Conference (Prior Notice)

3 REFERRAL PROCEDURES  The IEP Team Chairperson will receive the SSMT /referral packet and immediately schedule an IEP Team meeting  90 day timeline begins upon receipt of referral!  The IEP Team will review existing data (review SSMT/referral packet)  The IEP Team then makes the decision whether or not to conduct an evaluation and if so, have parents to sign DEC-2 for permission to test **DEC-5 AND MINUTES ARE REQUIRED AT THIS MEETING!

4 ELIGIBILITY DETERMINATION MEETING  Upon receipt of the testing packet check for completeness and schedule an IEP Team meeting  Complete eligibility worksheet for each catagory  Complete ALL components of the DEC-3 making an eligibility determination based on the data received  Draft an IEP  Complete the DEC-5, Minutes and if the child was determined eligible, the DEC-6

5 Reevaluation  Schedule the IEP Team meeting at least 90 days prior to the due date  Address ALL areas of eligibility  Address ALL areas on the DEC-7  Based on the information gathered the team will determine if additional data is needed to determine continued eligibility  Parent signs DEC-2 if additional data is needed  Complete a DEC-5 and Minutes **SEND REEVALUATION PACKETS TO YOUR COMPLIANCE PERSON!

6 ANNUAL REVIEW  Invitation to conference (Prior Notice)  Draft IEP  DEC-5  Minutes **PARTICIPANTS MUST INCLUDE: 1.THE LEA REP. 2.EC TEACHER 3.REGULAR ED. TEACHER 4.PARENT -REMEMBER…….ALL OTHER STAFF THAT WILL WORK WITH THE CHILD SHOULD BE INVITED AS WELL

7 COMPLIANCE SPECIALIST SHOULD ATTEND WHEN….  A change of placement is being considered  Eligibility determination  When adding or deleting services  Re-evaluation

8 STUDENT TRANSFERS From outside the System…..  Provide comparable services upon enrollment  Request the confidential folder via phone and fax  Schedule an IEP Team meeting immediately to review the data and to determine if additional data is needed  Update CECAS

9 Student Transfers Within our system……  Provide comparable services upon enrollment  Communication with Guidance and SIMS Operator is key  Request the confidential folder via phone and fax  Contact EC Social Worker  Update CECAS by changing the school and case manager name…..Do Not exit the child!

10 Transfers continued….. TRANSFERS OUT OF OUR SYSTEM……  Notify the EC Social Worker to pick up the folder once the request is received  Once returned to you place in an inactive file  Exit student from CECAS
