ACCIDENT / INCIDENT REPORTING (Employee Version)

Zero accidents or injuries are acceptable. The expectation is that none occur. Accidents / Injuries

What gets reported? How to report an Injury? How to report an Accident? Investigation Process Training Objectives

What is Important When an Incident Occurs + Secure the scene to make sure no one else will be injured + Seek medical attention if needed + Participate in the investigation + Help to determine the Root Cause Help in identifying changes that can be made to process / equipment / procedures

Timing + All Incidents should be reported immediately. Timing is critical. Allows for investigation of current conditions Provides an opportunity to make adjustments to prevent further exposure Prevents reoccurring accidents / injuries from happening + Any incident is an event and must be given attention that is needed

Injury Based Incident What happens after an Injury occurs? + After an injury happens, the event must be documented and learned from. You, as the injured employee, must report all injuries, no matter how slight, to the Coventry Nurse NT24 Line. The number is This is the very first step in the process. Next, you will need to contact your immediate Supervisor / Manager and inform them of the injury. There will be a Root Cause Investigation that takes place that you will be required to participate.

After an Injury + If medical attention is required, all documents from any doctor visits needs to be turned into your Manager / Supervisor. This includes all return appointments + These copies will be forwarded to appropriate Risk Adjuster.

Lost Time Injuries + A “Lost Time” Incident occurs when you are not able to report for your next scheduled shift as a result of the injury, this now becomes a Lost Time Injury for reporting purposes. + This type of case will have a documented Root Cause Investigation led by your Safety Manager. Your Manager / Supervisor will complete this form with your assistance This form can be found on the Safety MicroSite, under Safety Documents, Post Accident / Post Injury Documents. + Your participation in necessary to ensure we address any issues that could lead to another person getting injured.

Transitional Duty Transition Duty (Light Duty / Restricted Duty) + We want our employees contributing to the team and doing all he / she can to help their team. + Any time you can not return to your full job (prior to injury), we want to try an accommodate the restrictions to the best of our ability.

Documentation + Review proper forms Your Terminal manager will have the Injury Root Cause form to complete. This form and all related documents will be sent to the Risk Adjuster. + Timing is Everything! All injuries and accidents must be reported immediately. All injuries and accident must be reported, no matter how slight or significant.

Accident Based Incident + What happens after an Accident occurs? Employee is involved in an accident First call is made to 911 to get the proper People / Department to respond to the accident. (Whether this is Law Enforcement, Ambulance, Fire, etc.) Next call is made to Risk (800) , Option 2, to report the accident. All accidents need to be reported to Risk before you call your manager Employee calls their Manager Notifies them of the accident and supplies all relevant details Manager begins the Accident Investigation Process Employee would also call for any service needed such as a break-down, service help, etc.

Accident Based Incident + What happens after an Accident occurs? For a Fleet Driver: If the accident is deemed “Preventable” your Manager / Supervisor will conduct an Accident Investigation Call. Participants will be the Person Involved, Transportation Supervisor / Dispatcher, Terminal Manager, Operations Manager (or representative), and the Safety Manager. Purpose of the call is to gather all relative details to the accident. Call will take place within 72 hours of the accident. Terminal Manager will complete the Root Cause Investigation Form

Accident Based Incident + What happens after an Accident occurs? For a Shop Technician: If the accident is deemed “Preventable” your Manager / Supervisor will conduct an Accident Investigation Call. Participants will be the Person Involved, Shop Manager / Lead Technician, Regional Maintenance Manager, and the Safety Manager. Purpose of the call is to gather all relative details to the accident. Call will take place within 72 hours of the accident. Service Manager, Lead Technician or Reg. Maintenance Manager will complete the Root Cause Investigation Form

Accident Based Incident + What happens after an Accident occurs? For a Warehouse Associate: If the accident is deemed “Preventable” your Manager / Supervisor will conduct an Accident Investigation Call. Participants will be the Person Involved, Supervisor, Operations Manager, Vice President of Operations and the Safety Manager. Purpose of the call is to gather all relative details to the accident. Call will take place within 72 hours of the accident. Operations Manager will complete the Root Cause Investigation Form

Accident Based Incident + What happens after an Accident occurs? Training opportunities will be discussed. Manager will arrange for any training determined to be conducted. All documentation needs to be given to your Supervisor / Manager. Any police documents or anything else from the scene. Pictures are a necessity. Take pictures from the scene showing all details. All Documentation will be forwarded to the Risk Adjuster.

Accident Based Incident + Every preventable accident requires an Investigative Analysis Form be completed. Something to remember about the Accident Process: 99% of the time there is always an opportunity for some type of retraining.
