Working with Primary Care  Support to commissioning/contracts to develop and audit the LES  4 Health and Wellbeing Co-ordinators link into GP practices  Support for GPs with Annual Health Checks  Follow up work from Health Action Plans and Annual Health Checks  Support with making reasonable adjustments  Professional nursing advice and support  MDT Health Clinics

Acute Hospital Trust  Local Access to Acute Group – feed into regional group  Link in during admissions into hospital  Links with ECC and hospital discharge team  Membership on Partnership Board and Health Task Group  Quarterly monitoring includes hospital admissions

Health Promotion  Close working with Sports Development and 3 rd sector to develop healthy activities  Joint working with Health Promotion to ensure people with a learning disability are included in mainstream health promotion activities  Disability Health and Social Needs Assessment
