7 7 th grade immunizations l Tetanus Diphtheria and Pertussis booster (TDAP this is only one injection) l Meningococcal vaccination l 2 nd Chicken Pox vaccination l Don’t wait ! Records must be on file with school nurse. Providing proof of immunizations prior to Aug 5 th will speed up your schedule pick up process

8 Registration Form l 1 means the class lasts all year l ½ means the class lasts ½ a year (or a semester). l You must have 7 credits for the year l ½ + ½ = 1 Credit
