2 Some Barriers For Injury Prevention From Unknown Hazards  Barriers to Protect Against Unknown Hazards  Re-doing accelerator safety analyses every 5 years  Re-doing JRAs and FRAs every three years  Performing documented work planning (procedures, permits, safety committee reviews)  Obtaining feedback from workers (surveys, WOSH Committee, discussions during work observations)  Worker planned work (from SBMS SA): - From Step 3 + …? + Are conditions appropriate for work to proceed? + If at any time the worker is unaware of answers to the above questions he/she should suspend work and contact his/her supervisor or ESH Coordinator for assistance - From Step 4 + If the authorized work is not proceeding as planned, or new hazards are introduced or identified, the worker must re-evaluate the work or initiate Stop Work Authority if imminent danger exists 2
