7. Understand and provide information about more than 140 different LCHS programs Process approximately 18,000 referrals every year Understand and provide information about a variety of services external to LCHS Service Access team

8. Referrals to any LCHS program can be made by: S2S electronic referral system Email Fax Phone Walk in Incoming referrals

9. Service Access Officers (SAO) identify the needs of the client through an Initial Needs Identification (INI) process using the Service Coordination Tool Template (SCTT) On completion of the INI process and prioritisation, client consent is obtained and referrals sent to the appropriate services Initial Needs Identification (INI)
