2013 Operating Priorities This month’s Meeting in a Box focuses on: Financial Performance Growth Operating Model Portfolio Culture Transformation Operating Model Optimized Asset Portfolio Innovative Care Model Culture Transformation Financial Performance Each month, Meeting in a Box focuses on a few of the five 2013 strategic priorities. These priorities are the goals of President & CEO Sandra Bruce that have been cascaded down to our leaders so we are aligned and working toward the same goals. Operating Model – This is a large part of the Becoming ONE efforts as we work to redesign our organizational structure to truly become one system, fully aligned and integrated and achieving breakthrough performance. Optimized Asset Portfolio – This is everything the system owns – we are constantly looking to determine what we have and what we need to meet the needs of our patients. Innovative care model – This is the way we deliver care to our patients. This is about the new payment models and moving from a delivery of care that is based only on treating the sick and moving to a model that promotes health and wellness. Culture Transformation – A culture is based on values, beliefs and norms in an organization. At Presence, we have an opportunity to shape our culture to engage employees and physicians in creating an exceptional experience for every patient, resident and family member. Financial Performance – And this is our bottom line to ensure we can sustain our mission. The sixth item that will be discussed from time to time is any relevant Human Resources updates.

6 Financial Performance Update
Presence Health continues to operate below budget year to date through September. Hospital admissions continue to be unfavorable to budget by 9.5% and are down 7.7% compared to last year. As you know, strengthening our financial performance is an important part of our Becoming ONE efforts underway. Achieving the identified $188 million in savings over the next three years will enable Presence Health to better position ourselves for the changing health care environment to ensure we are meeting the needs of our patients. Our work is focused on increasing centralization and standardization with a focus on efficient & effective care delivery – this is what will create a lean and efficient health system. Through Becoming ONE, design teams have analyzed processes to develop standards across the System for achieving greater cost efficiencies and improvements. That work is finished and the focus now shifts from identifying opportunities to implementing approved initiatives that will reduce operational costs, streamline processes and shift work to best align with the emerging regional organizational structure. While some initiatives will provide quick returns and be implemented with ease, others will require an expanded rollout timeline and the cooperation of all employees Thank you for your continued support and cooperation in this important work.

7 United Airlines Added to SmartCare Network Clients
Growth United Airlines Added to SmartCare Network Clients 11,000 Chicago United Airlines employees and their family members can choose the SmartCare Network as a health care option effective January 1, 2014. What is the SmartCare network? It is a narrow network with a benefits design that incentives employer members to select the network to receive lower out of pocket expenses. This network was created by a company called Imagine Health, the developer of custom referral networks for employers branded as “SmartCare Networks.” This is the first network of its kind in Chicago. Presence Health has served as the anchor health care system for the SmartCare Network since January 2013. Other SmartCare Network clients include Home Depot and Walmart (totaling 29,000 members). The SmartCare network limits hospital and physician participation to certain markets in the Chicago area. Only hospitals and physicians meeting very specific criteria set by Imagine Health for quality, patient safety performance and patient satisfaction can participate. At this time, Presence Health markets included in the SmartCare Network are Chicago, Evanston, Des Plaines, Elgin, Aurora and Joliet.

8 Iroquois Memorial Hospital Update
The process continues and a decision has been made to move forward with a management agreement as a first step in forming a partnership. Terms and other details are currently under development and will be shared when the agreement is finalized near the end of the year. Presence Health is continually looking at the various components of its organization and the markets it serves to determine how it can best support its Vision of delivering clinical excellence, outstanding value and exceptional experience. Iroquois is a 49-bed hospital and 35-bed skilled nursing facility in Watseka, approximately 30 miles south of Kankakee, Illinois. The hospital has more than 300 employees. In June, it was announced that Presence Health and Iroquois Memorial Hospital in Watseka, Illinois had signed a letter of intent to form an affiliation. Partnering with Iroquois will help facilitate a strong, cohesive geographic presence throughout this region. Presence St. Mary’s Hospital (Kankakee), Presence United Samaritans Medical Center (Danville), Presence Covenant Medical Center (Urbana) and Presence Saint Joseph Medical Center (Joliet) all provide referral and clinical support options both north and south of Watseka. Iroquois County residents would benefit from being able to remain close to home for medical care, while having access to their choice of Presence Health specialists in nearby communities when necessary. As the financial challenges of health care reform continue to impact health care, the investment in strategic growth opportunities remains critical. The hospital also operates four primary care clinics in the surrounding area. The hospital began looking for a partner more than a year ago to ensure the sustainability of local access to high quality health care for the residents of the communities served by IMH, and to maintain jobs at the facility.

9 Working to Strengthen Presence OLR
Plan for the future: Our patients are our top priority.  A strong health care facility is needed in this community, but changes do need to be made so that Presence OLR can provide the very best care to those who rely on the hospital. Our goal is to develop a viable business plan for Presence OLR as a licensed hospital with an appropriate complement of inpatient services, comprehensive emergency services and ambulatory services. We must work together: Planning efforts must be supported by POLR doctors and medical professionals. Neighborhood community leaders must be involved. Elected officials are strong supporters and committed to POLR. Timetable: We have been considering a plan for the future for about nine months. It will take us time to implement the plan once a decision is made. Approval of the Presence Health Board would be sought at periodic junctures. We hope to come to our initial planning conclusions within the next three months. There have been questions as well as some misinformation shared about the future of Presence OLR. We talked about this in last month’s Meeting in a Box but want to reiterate some key points following the October board meeting.

The model is unique from typical regional structures as it integrates the entire leadership team, rather than just a “bridging CEO.” We believe it will be essential to develop a leadership team that owns the results for the combined ministry in order to truly maximize the outcomes. In each market this leads to various market synergies for keeping services within the health system and cost synergies as well. 11 Improving the Patient Experience
One way that we know we are improving the patient experience is through our patient satisfaction scores. Our target is to meet the 75th percentile goal by the end of the 4th quarter 2013. Keep up the good work – sometimes the difference between meeting or missing the goal is just one patient!
