2 NDSU - VPFA Training NDSU Policy: Section 708: Campus Maintenance and Service Requests http://www.ndsu.nodak.edu/policy/708.htm Request For Estimate Fill out Service Request Form, which can be accessed at: http://facilities-mgmt.ndsu.nodak.edu/maint_const/servreq.pdf Requires departmental funding. Requestor will be contacted by the assigned Project Manager in the order in which the requests are received. Project Manager calculates estimate, and requestor will be mailed a “budget estimate.” After departmental approval, return signed copy of estimate form to Facilities Management and your project will be placed in project queue.

3 NDSU - VPFA Training FAMIS Facilities Asset Management Information Software – Facilities Management Software which is an ancillary system to PeopleSoft. Billing is through an interface to PeopleSoft. Departments can request access to Discoverer Reports, which will then provide the detail of the charges in a particular billing cycle: http://facilities-mgmt.ndsu.nodak.edu/famis/index.html URL to run your reports once you have access: http://www.fac.nodak.edu/discoverer/viewer Training is provided quarterly, or by request. 4 NDSU - VPFA Training Space Move Authorization Forms These forms exist to collect the necessary information needed to maintain the University's facility space inventory. The information is updated on the FAMIS Facilities database for Space Use Analysis through the use of these records. This database provides information for campus planning; grant applications, building renovations, and ensuring building code compliance and utility usage. Please refer to the Space Move Authorization Form Instructions regarding whether to use Form A or Form B. When moving personnel from one room to another within the same department fill out the INTRA-DEPARTMENTAL MOVE FORM (ex: English dept. move personnel room 109 to room 229). http://facilities-mgmt.ndsu.nodak.edu/famis/FAMIS_SpaceMove_FrmA.pdf Form B. When moving a person(s) into another department’s space fill out the SPACE MOVE AUTHORIZATION FORM (ex. 6 NDSU - VPFA Training Motor Pool Reservations Phone in State Fleet vehicle requests to 231-9619, hours of operation 7:30 am to 4:00 pm Faculty, Staff, and Students must submit Reservation Form for billing purposes prior to vehicle release. Students traveling without faculty/staff member must also submit Student request for travel form. 8 NDSU - VPFA Training Key Requests Complete key request form with the following required information: NAME EMPL ID BUILDING ROOM NUMBER KEY CODE Building Key Control Official must sign off on all key requests It is the policy of the Facilities Management Department to issue no more than one key per person for each lock. 9 NDSU Campus Parking Office The Campus Parking Office is responsible for parking permits and parking enforcement on the NDSU campus The Campus Parking Office is within the Facilities Management Department located in Thorson Maintenance Center Hours of Operation: 8 AM – 5 PM, Mon – Fri Dept. #3310Phone: 231-5771 http://facilities-mgmt.ndsu.nodak.edu/parking/

11 NDSU Campus Parking Office Conduct permit sales throughout the year Annual, temporary, one-day permits Students, faculty, staff, visitors, vendors Online purchase system Revenue from parking permit sales are invested directly back into parking facilities; (i.e.: improvements, construction, resurfacing, striping, equipment, snow removal, signage)

12 NDSU Campus Parking Office Enforcement in cooperation with City of Fargo Parking Enforcement Officers dispatched through this office Citation fines are collected by City of Fargo Citation appeals are available online at the Campus Parking Office website Unpaid citations will result in a summons from the City of Fargo and possible impound
