38 Interventions Annual Form  When the priority school intervention model implementation begins, schools will complete the descriptions of interventions and outcomes, for each Federal Requirement and any permissible activities within.  Each year, the school will update their progress of implementation for that year and send the form to be reviewed. NOTE: All cohorts must reference the appropriate school improvement strand (and indicators) in their responses. Responses from this form should be copied and pasted into the appropriate section of the school improvement plan.

39 Interventions Annual Form Response should include: Initial Plan (Cohort VI)  Review the appropriate Wise Ways for each requirement.  Based upon the Wise Ways, describe the planned interventions. Responses must include the current status of the intervention and descriptions of planned interventions and outcomes. Note: Interventions may be planned and implemented across a three year period. Returning & Continuing (Cohorts I – V)  Based the school’s Initial Plan, describe progress/changes/revisions. Include appropriate references to their School Improvement Plan (SIP) indicators.

40 Interventions Annual Form Response should include:  Both a) and b) in your Initial Plan  If changes have occurred since their last response, include them in the appropriate response area for that school. For example: Additional goal setting training, added peer review of goals, documentation logs, change in midyear review meetings, etc.

41 Interventions Annual Form Response should include:  How school leaders, teachers, and other staff who have increased student achievement and high school graduation rates are identified and rewarded  How staff who have not improved, after ample assistance, are identified and removed

42 Interventions Annual Form Response should include:  Who determines professional development needs  How data are used in determining needs  How professional development is implemented and monitored

43 Interventions Annual Form Response should include:  Strategies for recruiting, placing, and retaining staff  Note strategies named in the requirement Response should include:  How data are used to identify and implement an instructional program as described in the requirement. Note: This requirement is about using data for instructional program needs. Student data use is addressed in requirement 7.

Response should include:  Plans that align with response to the three year goal in #l on the Leading Indicator Form - Increased learning time requirements will be discussed during Priority school training in the fall. Response should include:  Current and future mechanisms for family and community engagement

Response must include description/plans for support from Lead Turnaround Partner.  Cohort VI schools will describe state and district support and the plans/steps to identify the Lead Turnaround Partner.

52 School Improvement Planning Each group of schools listed below that did not meet state and/or federal accountability requirements per the Regulations Establishing Standards for Accrediting Public Schools in Virginia and/or the Elementary and Secondary Education Act of 1965 flexibility waiver are required to develop, implement, and monitor an improvement plan using the state-determined improvement planning tool, Indistar®. All priority schools will develop, maintain, and monitor their plan via the Transformation Toolkit.  Provisionally Accredited – Graduation Rate  Focus Schools  Priority Schools  Title I Schools that did not meet an annual measurable objective (including Title I schools rated Accredited with Warning)
