3 Senior Checklist REQUIREMENTCOMPLETEINCOMPLETECOUNSELOR SIGNATURE Be on-track to meet graduation requirements or have consulted with counselor if off-track Class requirements Credit requirements SL, Dr. Ed, PSAE, Consumer Ed Apply to 3 or more schools if on-track to graduate. Have a plan for next year. DEADLINE: JANUARY 15, 2014 Complete FAFSA or complete at least two scholarship applications if do not qualify for FAFSA DEADLINE: FEBRUARY 18, 2014 Maintain 90% attendance (Attendance Office; year-to-date attendance will be reviewed before end-of-the year senior activities)

4 Requesting Official Transcripts 1.Go to Curiehs.org 2.Click on Counseling 3.Click on transcript request 4.Complete form and pick up in College and Career Center 3-5 days after date of request ONLY REQUEST TRANSCRIPTS IF YOU ARE CLOSE TO FINISHING AND APPLICATION AND ARE DEFINITELY GOING TO COMPLETE IT. THE FRIST 3 ARE FREE; THE COST IS $3 PER TRANSCRIPT AFTER THE THIRD. APPLICATION FEE WAIVERS ARE ALSO AVAILABLE IN THE COLLEGE CENTER FOR STUDENTS WHO RECEIVE FREE OR REDUCED LUNCH.

5 TRANSCRIPTS, TEST SCORES, & WAIVERS When you request a transcript, your ACT score will be on the transcript. You can ask for an application fee waiver in the College Center when you pick up your transcript. Some schools, such as University of Illinois at Urbana, have their own fee waiver. The common application also has a fee waiver. Please let your counselor know if you need a special fee waiver for any of the schools to which you are applying. University of Illinois at Urbana, University of Wisconsin at Madison and Northwestern, University of Chicago and other highly selective schools will only accept test scores from ACT and will NOT accept scores that have been copied to your transcript. If you are applying to these schools and didn’t request for your scores to be sent there while taking the ACT, then you must go to act.org and request that your ACT scores be sent to the school. Each score report costs $16. University of Illinois at Champaign doesn’t want your official transcript until after you are accepted and graduate. You must self report your academic history for UIUC using your unofficial transcript.

7 If you are completing the Common Application, Make sure you list your counselor for the Education Section AND as the assigned recommender for each school to which you are applying For each school, make sure you select the appropriate deadline for which you want to be considered After submitting your common application, you can still edit your personal statement up to three times Although transcripts are submitted electronically, you must order and send paper transcripts so that your test scores are sent to each school Although the common application is one application, you must still press submit for each school to which you are applying You will only be allowed to submit once you have a green check mark next to each category on the application

Click on, “Freshman Application” and then, “Create an Account” Priority Deadline: November 1, 2013 Final Deadline: January 2, 2014 Middle 50% of students admitted last year had between a 27 and 32 ACT. University of Illinois at Chicago 1. Apply through the Common Application at https://www.commonapp.org/Login https://www.commonapp.org/Login Early Action Deadline: November 1, 2013 Final Deadline: January 15, 2014 Middle 50% of students admitted last year had between a 22 and 27 ACT.

On the right, click on “Apply Online” 4. Click on “Create Your Account” and begin Requirements: Will graduate from high school in top half of your class or minimum ACT composite of 19 or GED If you don’t meet these requirements, you can apply through different Academic Success Programs (Proyecto Pa’Lante or Project Success); check out these options at neiu.edu. Complete regular application and check box for consideration to these programs Deadline: July 1, 2014 Northern Illinois 1. Go to http://niu.edu/index.shtml http://niu.edu/index.shtml 2. Click on “Admissions” and then “Undergraduate Admissions” 3. Click on “Freshmen” 4. Click on “Apply Now” 5. On right, click on “Begin Application” 6. To create account, click on “sign up for one here” Middle 50% of admitted students have 2.85-3.47 GPA, rank in the 50-78 percentile of class and have an ACT composite of 20-25 CHANCE PROGRAM: You can apply through the CHANCE Program (must write a personal statement) if you don’t meet one of the admission requirements. Eastern Illinois University 1. Go to http://eiu.edu/ http://eiu.edu/ 2. Click on “Admissions” 3. Click on “Apply Now” and then “Apply Online” 4. Create an account and begin Average Stats of Admitted Students: ACT-21 & GPA-3.09 Deadline: Rolling Admissions

Click on “Undergraduate Application” 5. Register and begin Info about Admitted Students: Average ACT-21; Average GPA-3.0 If your GPA falls below 2.63, you should write a personal statement about your academic goals, how you plan to carry those out at WIU and any circumstances that have caused your grades to be lower. You may be accepted through the Academic Services Program. Most students accepted through this program have an ACT higher than 16 and a GPA higher than 2.0. Fill out the regular WIU application and include your personal statement. Letters of recommendation may also help. Southern Illinois University Carbondale 1. Go to http://siu.edu/ http://siu.edu/ 2. Click on “Admissions” 3. Click on “Undergraduate Admissions: 4. Click on “Apply Now” in center of page 5. Click on “Apply by May 1” for Fall 2013 6. Click on “Apply Online Here” 7.
