7 Service User Reference Group Shaping your service We value your ideas and opinions We value your time Come and have your say about your Service! We pay expenses and we meet on the first Wednesday of every month at 3pm. Next meeting at: Beeston Hill Medical Centre

8 Service Development Ideas We contributed to the development of resources We worked on ideas for service improvement

9 S.M.A.R.T.E.R Friend Get S.M.A.R.T with the help of a S.M.A.R.T.E.R friend. Help with Your S.M.A.R.T. friend will liaise with your key worker to help you achieve set goals Budgeting Employment Education Healthy Eating Time Management You will be able to contact your S.M.A.R.T friend at times when you need them. Your S.M.A.R.T friend will be an ex-service user who understands your needs. They will be able to signpost and enable you to choose goals that suit you. For all activities you attend with your S.M.A.R.T Friend you will receive points. Each month, these points will be used to reward your commitment and motivation. A S.M.A.R.T Friend will encourage and empower you to achieve your personal S.M.A.R.T goals. PECIFIC MEASURABLE PECIFIC MEASURABLE ACHIEVABLE REALISTIC TIMELY PECIFIC MEASURABLE
