4 SF CW HSP Housing First model that assist CalWORKs families in quickly obtaining permanent housing paired with JOBsNOW. Subsequently, wrap-around services are provided to foster housing retention and to address barriers that impact the recipients’ path towards self-sufficiency. Model: Housing first paired with JOBsNOW and wrap-around services. Process Flow: Referrals or client self-identification for HSP Family Screening or Assessment Process for HSP HSP Intake and determination of HSP Eligibility Housing identification, matching and placement process, performed by contract provider Housing Case Management that focuses on rental compliance and retention, and then coordination with JOBsNOw and wrap around services. Housing Services: Gradually decreasing rental subsidy that can start up to $ Goal is to decrease subsidy at every renewal while also increasing family’s income. Court-order eviction assistance and costs associated to prevent eviction process Move-in fees and any furniture deemed necessary to the family Housing search and placement services Housing barrier assessment and housing case management to foster housing retention Legal services and credit repair Other essential related costs may be covered on a case-by-case basis. Wrap Around Services: JOBsNow-unsubsidized, subsidized, Public Sector training with SF County, Community Jobs; Individual training Internship Vocational English as a Second Language Immersion Program CalWORKs benefits such as cash, food, and medical Employment services that includes job preparation services, training, subsidized employment, and unsubsidized employment Early Engagement: Robust Appraisal, Family Stabilization and/or SSI Case Management Behavioral Health and Domestic Violence Services Ancillary or supportive services not limited to child care and transportation Money management classes Free tax preparation Social Work support-as needed, different from Housing case management SF Presentation at CDSS CW HSP Conference in Sacramento 2/26/15 4/20/2017

9 SF CW HSP Guidelines Rental subsidy is up to $ , and can cover up to 100% for the first 2 months. A 30% subsidy decrease starts on the 3rd month, per income. Housing Plan is created and reviewed for progress every 3 months. Income and rent amount are major factors of consideration. Existing barriers and issues are considered including its occurrence and length- if possible. Associated move-in costs determined essential and one-time only can be covered. Move-in furniture limited to bed, dresser and kitchen table, chair, and couch with a limit of no more than $350 per item. Wage Subsidy Program provides work opportunities to low-income San Franciscans while offering employers a wage reimbursement for new hires.  Un-Subsidized Jobs Wage Subsidy (WS): Permanent jobs in private or non- profit business with an average wage of $16.00/hr. Wage Reimbursement for up to $5000 for employers. Public Service Trainee (PST): City of SF Employment earning $12.98/hr, 32hrs/wk for 6 months Individual Training Internship Program (ITIP): Paid Internship in a non-profit agency earning $11.03/hr 32hrs/week for 6 months Community Jobs Program (CJP): Paid Internship earning $11.03/hr 25hrs/week + professional development classes Vocational English as a Second Language Immersion Program (VIP): ESL Internship + Paid Work Experience at Level 3 or Level 4 earning $11.03/hr, hrs/wk Note: wage amount not an accurate reflection of current wages due to recent increase SF Presentation at CDSS CW HSP Conference in Sacramento 2/26/15 4/20/2017
