4. PRACTICAL ISSUES Confirmation of Study Period of study: Feb 09, 2015 – May 15, 2015 Exchange students can finish their exams by May 15, 2015. Between May 15 – June 26, 2015 (exam period) ONLY in case of registered exams in ISIS during this exam period (last day of students’ duties = last exam) ! Final grade: 4+ ( 50 – 59 points, fail with the possibility of a repeat attempt)

6. PRACTICAL ISSUES Learning Agreement Agreement between 3 parties –Student, home uni and host uni Student agrees to study the courses Home uni agrees to accredit the courses in the full degree programme Host uni confirms that the courses are in line with its course catalogue

LEARNING AGREEMENT WHO IS RESPONSIBLE Your home university issues ALL documents How to complete your LA: 1.Responsible person at home institution has to agree with courses 2.Fill in your personal data, courses and sign it 3.Signature of institutional/ departmental coordinator 4.Signature of host university coordinators (Daniela Slámová)

8. CHANGES TO THE LA All changes has to be approved by responsible person at home university first Write all final changes to Section to be filled during the mobility Forward us the approval of changes by your home uni or bring it to RB547 to have it signed

9. PRACTICAL ISSUES Learning Agreement Needs to be –Signed by the home uni first –Brought or sent by email to Exchange office –Amended when a new course is added

10. Transcript of records Transcript of recods will be send automatically to your university once all your grades are on ISIS
