3 3 Definitions and terms (1/3) Crew - a group of employees on shift, consisting of a shift leader, an engine driver and shunters who make ​ the placement of wagons, their movement at the loading or unloading, as required by the head of transport and the requirements of production plants. The shift leader – is designated person by the employer in charge of business operations and logistics services on the shift. He is responsible for ensuring safety on shift and he is responsible to the Head of Transport. It can be an engine driver or a major shunter on the shift. The engine driver - a person who is appointed to drive vehicles - locomotives, should complete a proficiency test periodically ) and safety training at least annually. The shunter - a worker who provides the movement of wagons, coupling and uncoupling. He should pass a proficiency test and safety training at least annually.

4 4 Definitions and terms (2/3) Railway siding - a railway track that serves its own needs of the operator and is connected to track national, regional railroad or other rail siding. Procedures and work instructions and driving instructions – It is a set of documents (technological, technical, work, safety, environmental), which the crew must follow and which must be followed. Parking - Safe security (brake) shutdown and securing engine, train or individual wagon to prevent their movement Wagons coupling and uncoupling – it can be done manually or automatically Switching and shunting - movement of train on rails and using routing switches in a predetermined location.

5 5 Definitions and terms (3/3) Dispatching - logistics control movement of trains and wagons in the plant and their loading and unloading Loading and unloading – this operation is managed dispatching and providing crews of dispatch hall and stock hall or stock area

6 6 FPA – L2Q1, L2Q2 Crew Roles and Responsibilites Has one person been appointed as responsible for Rail activities? Is his/her mission defined and properly documented? Has he/she the authority, skills, training and means to ensure safety for rail activities? Have the following positions been defined as well as the corresponding safety rules they have to abide by? Driver, leader of operations, hitcher, switchman, level crossing keeper, maintenance of staff, look-out signaler, dispatcher? Crew must be trained on testing on a regular basis in detailed technological regulations, working procedures, safety rules, providing of first aid. Drivers and shunters must be trained, competent and tested to be qualified.

7 7 Alcohol and drugs – FPA L4Q4 Are there awareness programs about consequences of alcohol and drugs consumption on heald and safety? Alcohol and drug abuse on the railway is strictly prohibited! The employer must ensure that checks whether workers are not under the influence of alcohol or drugs during their work on the railway.

8 8 Emergency drill on the railway situations FPA L4Q2 Are yearly emergency drills conducted to test rail emergency response plan? Recommendations and good examples ! In AM Karvina are conducted annual exercises with firefighters and emergency medical service

9 9 Driving Instructions FPA – L2Q3 Have rules for the line-up and positioning of railmanuever teams been defined (driver, leader of operations, shunter) and written down (depending on the manuevers to be performed)? The shunter and engine driver watch the route when handling railway vehicles; the engine driver uses a warning acoustic device at level crossings and the shunting master and shunter make sure that no employees or vehicles (trucks, fork-lift trucks, lancers, cars) get in the way of the vehicle, especially in blind sections between production halls – great danger of fatal accident!!!

10 10 Driving Instructions - national railway or another company FPA – L2Q3 Have rules for the line-up and positioning of railmanuever teams been defined (driver, leader of operations, shunter) and written down (depending on the manuevers to be performed)? When national railway company or another railway carrier arrives at the company plant railway siding, the company’s rail transport employees will cease their transport and handling activities and wait until the employees of another railway carrier carry out necessary addition, coupling or uncoupling of their vehicles and the vehicles of another railway carrier leave the plant railway siding. After that they can resume their work.

11 11 Procedures and Work Instructions - PPE FPA - L1Q13 Have the necessary PPE been defined on the basis of a HIRA for all the rail activities? Are they provided to workers and worm? Are special conditions as outsite work and work by night taking into account? Does rail activities staff wear ankle safety boots? Do they use clean high-visibility and thermal clothing when necessary? All rail transport employees shall use approved personal protection equipment for the respective job - high-visibility clothing, safety helmets, goggles, ankle safety boots, etc., according to the PPE regulations.

12 12 Parking FPA L1Q6 Railway locomotives and traction vehicles (locomotives and wagons) must be weaned safely and secured against movement and sudden rolling. The delivery, loading or unloading of wagons shall be provided with a brake and wedges. It is not allowed to crawl under railway vehicles without a reason, cross the track behind the railway vehicles at a distance shorter than 3m, or 10’ it is not assured that the railway vehicles cannot be set in motion, or cross the track right in front of or behind moving railway vehicles. Have safety rules written on the basis of the HIRA for rolling stock parking (including braking and wedging rules)?

13 13 Parking and walk on tracks FPA L1Q9 It is not allowed to crawl under railway vehicles without a reason, cross the track behind the railway vehicles at a distance shorter than 3m, or 10’ it is not assured that the railway vehicles cannot be set in motion, or cross the track right in front of or behind moving railway vehicles. min.3m Is it forbidden to walk on tracks? Have safety rules been written for situations when duties require people to go on or near the line (on or within 3 meters / 10 feet of a railway track)? Have those rules been written on the basis of a HIRA? Do those rules ensure the concerned people are protected from rail movements by track isolation using derail or switch locks? min.3m

14 14 On and Off Trains; FPA L1Q4 You must not get on a moving rail vehicleunless it is absolutely necessary, and then only if you can do so safely. You must not ride on the steps of a locomotive or vehicle or ride on a hand trolley or any other vehicle not designedfor this purpose. It is never permissible to mount, dismount, or cross over movingequipment. Procedures for getting on and off trains must include the use ofengineered sill steps, handles and ladders/steps placed so to prevent injury to legs, feet and hands. When riding, getting on and getting off rail equipment, three point contactmust be utilized at all times. Person operating rail equipment should be mounting and dismounting moving equipment only where it is absolutely necessary. Employees are prohibited from stepping on coupler or shanks of couplers between coupler head and car sill. On hopper cars fall protection must be utilized whenever there is a danger of employees falling from the car into a bin or hopper below the car or being engulfed by material in thecar.

15 15 FPA L1Q4 When shunting railway vehicles, before they start shunting, the employees shall make sure visually that carriage footsteps and grab handles are in good condition, that there are no overhanging or projecting parts, that the draw-gear, buffer gear and uncoupling rod of the railway vehicles are in good condition and test the operation of hand brakes. Is not permissible to step up, step down or stand on the break platform, steps, lateral footsteps of the railway vehicle, either moving or at a standstill, if one cannot hold on the vehicle grab handle firmly. Not this way ! Have safety rules for the access into or from vehicles (locomotive and wagons) been written? - ban on getting on and off while going, - ban on going between 2 vehicles, - ban on getting off a vehicle face to space - obligation to get on and off using engineered sill steps, handles and ladders / steps, - ban to cross the line between standings cars or standing cars and bumpers where separation is less than 3 meters (10 feet) unless the track is isolated

16 16 Coupling and Uncoupling FPA L2Q3 Railway vehicles can only be coupled and uncoupled when at standstill; stepping in front of or between moving railway vehicles for their coupling and uncoupling is forbidden and will be regarded as gross violation of work discipline and safety regulations. The shunter and the engine driver – they must see each other when they transmit optical or hand signals. When railway vehicles are approaching other standing vehicles, the shunting master or shunter on the side where the caution signals are given must make sure that the employee performing coupling and uncoupling does not step on the track before the railway vehicles stop. When stepping between the standing vehicles, the “Compress” signal may only be given if their bumpers are in contact. It is forbidden to handle vehicles (wagons) at loading ramps, if there is a loader in the vehicle (wagon), or if a vehicle (wagon) is being loaded or unloaded by a crane or a transport handling truck. When handling and shunting tank wagons with chemicals the employees must take extra care. (sulphuric acid, lime hydrate,…) Have rules for the line-up and positioning of rail maneuver teams been defined (driver, leader of operations, hitcher) and written down (depending on the maneuvers to be performed)?

17 17 Coupling and Uncoupling - Risks During work of a shunter and an engine driver there is frequent coupling and uncoupling of cars manipulations. Main risk of injures: Tripping on rail Squeezing between bumpers Back injury Finger pinching bumpers screw

20 20 Switching, Shunting FPA L1Q5 The rail transport employees must use: signal flags hand-lights means of communication – walkie- talkies must follow the agreed signals keep these means of communication in good technical condition discard them in case of damage and replace them use the communication signal light in poor visibility – in haze, in the dark, in rainy weather. are rules in place regarding “kicking” rail cars? Have safety rules been written on the basis of the HIRA for shunting operations?

21 21 Loading, Unloading and Dispatching FPA - L1Q7 Have safety rules been written on the basis of a HIRA for loading and unloading operations on and from wagons? Do those rules take into account the vehicles gross weight, the necessary load sharing out, wedging and stowing in the wagon? The shunting master and shunter shall secure the railway vehicles at loading ramps and detached vehicles against motion by approved stop rail blocks, only approved stop rail blocks must be used. Before loading the car to be secured against motion. Employees who load cars must comply the wagon weight limit - the table on each wagon. It is forbidden to overload the wagons or to load wagons unilaterally. The load must be uniformly loaded and secured to prevent shifting of the loaded material.

22 22 Audits must be planned and formalized Procedures for handling emergencies, like isolation of tracks and other safety rules must be frequently audited and the results of these audits should be shared with everyone along with their impact on the rail safety plan. Audits should involve leaders and workers Audits should be conducted on routine emergency situations along with major emergency planning activities Table top drills should be conducted annually with for rail operations Recommendations and good examples ! FPA questions -Audits; L3Q2 and L3Q3 Is the responsible person for rail activities always informed prior any work when isolation is necessary by the supervisor or his designee? Is a HIRA always performed before starting any work? Is a safety plan defined on the basis of the HIRA including the necessary safety actions to protect workers from rail traffic movements? Are formalized audits regularly performed to check compliance with the safety plan (particularly concerning the isolation procedure respect)?
