B ALTIMORE C ITY P UBLIC S CHOOLS Shipment of Materials Inventory test materials upon receipt using Scan Tracks and the handheld scanner. Always keep a copy of the original packing slip and the discrepancy log for future reference. Make sure numbers agree with what is on the vendors packing slip. Make note of any discrepancy directly on the packing slip, initial it, and complete Appendix C in TACM. Notify OAA of the discrepancy immediately. 16 Initial school shipment: September 18-22 Test materials and preprinted/generic student Id labels Add Order and Short Order Shipments September 18-October 10

B ALTIMORE C ITY P UBLIC S CHOOLS Ordering Make-up Test Materials For the October 2014 HSA and Mod-HSA paper testing, there will be one test form used on the primary testing date. This same form will also be used with students taking the test on the scheduled make-up date. 17

B ALTIMORE C ITY P UBLIC S CHOOLS Applying Labels Apply Student ID Labels to Answer Sheets When Pre-printed student ID labels are used STCs do not need to bubble in any information on the Answer Sheet covers. When generic ID labels are used STC/Test Examiner must bubble in all student information on the Answer Sheet covers, including the State ID number. DO NOT apply labels on top of each other! 18

B ALTIMORE C ITY P UBLIC S CHOOLS Distribute Test Materials Test Archive Documentation: Student Tracking Form (see Appendix J of TACM) Record distribution of HSA and Mod-HSA Test Books and Answer Sheet (see Appendix D of TACM) 19

B ALTIMORE C ITY P UBLIC S CHOOLS Student Accommodations STCs and other appropriate school personnel should verify and arrange test accommodations as soon as possible. Maintain a master list of all students and their accommodations, be prepared to submitted to the OAA upon request. Know what accommodations each student will be using and how to administer them properly.  See Maryland Accommodations Manual for Instruction and Assessment (MAM). 20

B ALTIMORE C ITY P UBLIC S CHOOLS Best Practices for giving Accommodations Schedule students testing with accommodations first. Group no more than five students for verbatim reading of the entire selection. When providing the accommodation of verbatim reading of the entire selection, students must be placed with other students receiving the same accommodation.  See Appendix T in TACM for Verbatim Reading Rules for Mathematics Items (TACM pages 98-99) 21

B ALTIMORE C ITY P UBLIC S CHOOLS STCs Reminders Be available to answer questions that may arise Monitor the test administration Make sure that personal items are not being used and that prohibited aids are not displayed Ensure that all school personnel adhere to the security guidelines Make sure that Test Examiners have used the Student Tracking Form 23

B ALTIMORE C ITY P UBLIC S CHOOLS Paper Test Reminders Review and be familiar with IEPs before testing begins so that all required accommodations are provided. Be sure each paper test-taker is using his/her correct preprinted ID label. Be cognizant of students with similar names so that wrong demographic information is not accidentally applied to student answer sheets. Be sure that information recorded on student tracking forms is accurately and entirely completed. The document should have students name and signatures, lithocodes, test examiners signature, and dates The document should clearly and accurately identify absentees and invalidations 24

B ALTIMORE C ITY P UBLIC S CHOOLS Stop the test for that student and record the amount of time remaining. Notify STC immediately. If student is able to return the same day, resume using same paper test. If the student is able to return to finish the test later in the testing window, he/she must resume paper testing on the state-mandated make-up date. If the student is not able to return to finish the test before the close of the testing window, the STC and OAA must then together determine if the student’s test will be returned for scoring or invalidation. Irregularities During Testing 25 Refer to pgs 22-23 of the TACM for complete procedures.
