Federal Aviation Administration N JO Operational Error Reporting, Investigation and Severity Policies Target Audience: Terminal Operational Personnel Effective June 25, 2007

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ Background Terminal operational personnel were previously trained on N JO , Operational Errors Severity and Proximity Events, which took effect 2/26/07 N JO , which took effect 4/30/07 canceled N JO N JO is the latest change and will become effective on 6/25/07. It will cancel N JO

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ Background (cont) N JO changes: –The major change for the control workforce is that Proximity Events (PEs) are no longer considered Operational Errors (OEs) N JO changed Category D OEs to Proximity Events, but a Proximity event was still considered an error –Training documentation and reporting requirements for PEs will be different than those for OEs and ODs –The definition of an OE has changed

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ Proximity Event (PE): Definition A loss of separation minima where 90 percent or greater separation is retained in either the horizontal or vertical plane, and does not include any violation of wake turbulence separation minima. A Proximity Event is not an operational error.

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/07 5 5

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ Operational Error (OE): New Definition Less than 90 percent of the applicable separation minima results between two or more airborne aircraft… »N JO Used to say “Less than the applicable separation minima…”

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ PE: Employee Notification Employees are not required to be verbally notified that they have been associated with a possible PE Employees are required to be verbally notified that they have been associated with a possible OE/OD

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ PE: Training Documentation No requirement for a separate page in Section VI of the The following training items must be entered in the without reference to the PE –Any associated remedial or skill enhancement training –Any associated recertification

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/ PE: Different Reporting Form Possible OMIC/OSIC/CIC responsibility Proximity Events will be reported on a different reporting form than the used for OE/ODs – The PE reporting form will be Faxing Form to the ROC/WOC within 4 hours of the PE. Verification that the fax was received is the only required notification for PEs.

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/07 10 Previous Method of Determining OE Severity Composite Slant Range: The mathematical result when the percentage of horizontal separation retained and the percentage of vertical separation retained is combined to create a single number that best represents the resultant airborne separation.

Federal Aviation Administration Briefing to Terminal Operational Personnel N JO , effective 6/25/07 11 New Definition of Slant Range Slant Range: The straight line distance between 2 aircraft. https://aap.faa.gov/
