Person Centered Planning Threeways values all its users as individuals, and believes that however severe their disability, they should be encouraged and enabled to live as normal and fulfilling a life as possible. The Person Centred Planning is central to the services that the home provides to users and encompasses the main areas of: Assessment of the Service Users needs; Planning through the Lifeplan meeting (a forum where the Service User, their Keyworker, parents, carers and professional staff gather to develop a plan to meet the Service Users strengths, needs and wants; followed by the Implementation of the plan and later its Evaluation and Review. Evaluation is on-going appraisal and reappraisal of the issues raised by the service user, their advocate and the multi disciplinary care team and occurs every six months or more frequently, according to the service users needs. All service users are offered opportunities for making choices in their daily activities.
