1 (A) Extrapyramidal signs seen here such as torticollis, inability to speak, and trismus were the presenting complaints in this girl who was brought to the emergency department from the school and there was no history available. Based on her clinical findings, phenothiazine toxicity was suspected. (B) Dramatic improvement after IV diphenhydramine. Extrapyramidal signs are very frightening to caregivers (especially in a previously healthy child without any history of ingestion), and following the dramatic improvement, treating physicians are often lauded as heroes. Subsequently we learned that this patient’s uncle was receiving mesoridazine for nervousness, and the patient confessed to taking her uncle’s medication. (Photo contributor: Mark Silverberg, MD.)
