5 Test Form Design ELA: 70 questions in four parts –Parts 1, 3, and 4 include a Listening section, which consists of one audio recording (available on an audio CD) with two associated questions Math: 70 questions, in three or four parts –Grades 3-5 have four parts, that mimic the layout of the computer- based assessment –Grades 6-10 have three parts, with the third part containing all calculator inactive items for easier and secure administration –The number of questions varies by grade and form: Science: for grades 4, 8, and 10 –Grade 4 has 55 questions in three parts –Grades 8 and 10 have 65 questions in three parts Part 124 – 27 questions Part 224 – 27 questions Part 316 – 22 questions

6 New Materials for 2015-2016: Science & ASL Science booklets for grades 4, 8, and 10 –Each student will receive a periodic table reference sheet with their test booklet –Periodic table reference sheets will be shrink-wrapped with the test booklets, three to a package (three test booklets, three single period table sheets) Orders requiring additional single test booklets will receive single periodic tables shrink-wrapped with each individual test booklet. For instance, an order of 8 science tests would receive two packages of three booklets and three periodic tables each, shrink-wrapped together, plus two single test booklets each shrink-wrapped with a single periodic table ASL DVDs –Video recordings of an ASL signer are available for all listening sections in the ELA assessment.

7 New Materials for 2015-2016: Listening Sections ELA Listening Sections –There are three listening sections on each ELA assessment –Each listening section consists of 1 audio recording and two associated items –Listening recordings and items will be the same on both the A and B forms for each grade, and will be in the same location on all forms: Part 1: Items 24 and 25 Part 3: Items 14 and 15 Part 4: Items 14 and 15 –You may administer the listening items in the following ways: Provide each student with a CD player and headphones to use when they reach the listening items Administer all listening items to all students in the same grade level as a group, at the beginning or end of the test session

10 For paper-based AMP: –In the TEC file, input P for paper-based for the test type For large print: –In the TEC file, input P for paper-based for the test type –In the PNP, go to the “Other Supports” tab and under the options for “Supports Provided by Alternate Form,” select “Alternate Form – Large Print Booklet.”

11 For Braille: –In the TEC file, input P for paper-based for the test type –In the PNP, go to the “Language and Braille” tab and select “Braille” For ASL: –In the TEC file, input P for paper-based for the test type –In the PNP, go to the “Language and Braille” tab and click “Signing Type”. Leave the drop down on ASL.

12 For paper-based as an accommodation: –In the TEC file, input P for paper-based for the test type –In the PNP, go to the “Other Supports” tab and under the options for “Supports Provided by Alternate Form,” select “Alternate Form – Paper and Pencil.”

13 Audio CD of Text-to- Speech The Text-to-Speech accommodation for the paper-based AMP includes an audio CD recording of the TTS feature with the test booklet kit In the PNP, go to the “Audio & Environment Support” tab, select “Spoken Audio”, then Math and Science Or Math, Science and English Language Arts

15 ASL Video Accommodation includes a DVD recording of an ASL signer for ELA listening sections ONLY In the PNP, go to the “Language and Braille” tab, select “Signing Type”, then choose “Default (American Sign Language)”

17 Student Label –Labels generated based on KITE TEC files –Two-three white labels per student, may be used with either test booklet (three for those students in grades 4, 8, and 10 receiving the science assessment) –Attach label to the back cover of test booklet –If a label is not provided, student demographic information must be entered and bubbled –Important to ensure label is attached in the correct place Test Materials - Labels

18 Special Circumstances –Last year, codes were recorded and sent to AAI then entered into Educator Portal –This year, the codes will be available on the demographic page in the test booklets and will be scanned in to the system: –Absent, Student Refusal, Parent Refusal, Medical Waiver, Invalidation –Teacher Use Only box: this box will allow the teacher to note if a student has been provided accommodations as noted in their IEP NEW Special Circumstances Codes

20 Outbound Shipment –Sent to the District Test Coordinator (DTC) –DTC materials packaged separately from school materials –School materials will be packaged individually –Test booklets shrink-wrapped in packages of 3 –Test forms spiraled (purposely mixed for security) –All materials must be inventoried and accounted for –If anything included on the packing slip is missing, contact customer service immediately Materials Shipments

21 Major Steps for District Test Coordinators 1.Inventory and organize all assessment materials returned by the Building Test Coordinator; verify and complete all required forms 2.Ensure all materials are returned 3.Package used test booklets and related forms; arrange for pick-up of scorable materials 4.Package unused test booklets, including accommodated materials; arrange for pick-up of nonscorable materials 5.Ensure each box with the correct Questar label **NOTE: All materials must be shipped by April 15, 2016** Materials Return

22 School Information Sheet –Building Test Coordinator marks the number of used test booklets being returned for each grade assessed –“Pre-slugged” with each school’s information –District Test Coordinator verifies the number of test booklets being returned is correct –Pack keeping the School Information Sheet on top of the corresponding materials Materials Return

23 District Return Kits –Materials Return Instructions –Return Summary/Transmittal Form –Questar Return Shipping Labels –UPS Shipping Labels Scorable vs. Nonscorable –Scorable materials include all used test booklets with student responses to be scored –Nonscorable materials include unused test booklets or test booklets that should not be scored Materials Return

24 Return Summaries –District Test Coordinator completes a scorable and nonscorable return summary for the return shipment –Record the total number of each material being returned –Sign and date –Make a copy for district records –Place on the top of package being returned –One form per shipment Materials Return

25 Return Shipment –Materials will be returned to Questar by the DTC via UPS pickup –Return materials in the same boxes the materials were sent –Make copies of forms for district records –If there is not a standard UPS pick-up at your location, contact UPS to arrange **NOTE: Manuals are not returned** Materials Return
