3. DG Cargo : 1.Send DG DECLARATION FORM (click HERE for sample )to APL. Booking will be released within 1-3 days once having approval from related parties.HERE 2.Please send DG DECLARATION FORM (with signature & stamped), to APL before laden drop off (by 09:00 on Friday for vessel depart on Sat/Sun/Mon). Kindly note without these documents your shipment can not be loaded on board the vessel. Container can only be loaded on board once having approval from OPS team. Pls be noted container must have DG placecard - Be not less than 250mm x 250mm - Correspond to the label for the class of the D&H goods in transport unit with respect to colour and symbol - Display the number of the class, as appropriate, in the lower half in digits not less than 25mm high - Sticked on the 4 faces of container (Front, Door, Left and Right side panel) Approval granted is subject to SHIPPER's RESPONSIBILITY : Compliance of shipment with all applicable respects of the IMDG code including (but not limited to) documentation, packaging, markings, labelling/ placarding and any other national or international governmental regulations which may apply including co- loading DG application to ensure that it meets IMDG requirements. Notes for DG Tank: Tank certificates not required with DG application as attachment when DG apply for APL operated vessels Tank certificates need to attach with DG application when DG apply to loading on partner’s vessels.

Container can only be loaded on board once having approval from OPS team.
