Patient Scenarios and Simulation Technology in Mental Health Nursing Marian Traynor & Fiona Martin (QUB)

Simulation using high fidelity technology already established in undergraduate curriculum for adult and children's. Challenge was to develop and pilot simulated scenarios for mental health field of nursing. NMC Standards for preregistration nurse education (2010) – generic competencies and field specific competencies Background

To promote the importance of addressing the physical health needs of an individual with a mental health illness Promote an interactive/team approach to student learning through the use of “real life” scenarios To encourage the development of a practitioner with the ability to think critically and analytically. To facilitate students in the use of peer and self-evaluation as means of promoting learning Objectives

Purpose built simulation suite Two case scenarios: Patient with Head Injury/ Patient with Asthma Second year mental health nursing students (n=17) 3G simulator Each scenario lasted no longer than 20 minutes followed by 30 minutes debrief. Scenarios video recorded to assist with the debrief. Complete a post 20-item questionnaire Methods

Environment

SimMan Realistic airway Pharyngeal swelling Laryngospasm Respiratory movements Synchronised breath sounds Pupil size Pneumothorax / chest drain Apnoea Sounds – cough/vomit etc Pulse and BP SaO2 Live defibrillation

What happens? Orientation Scenario – assessment (ABCDE) / management – team working / communication Reflection Feedback

Results 100% agreement that scenarios were a useful learning experience 100% agreement that the simulation exercise would have a positive influence on ability to perform in clinical practice 100% agreement that the scenarios using the HPS was a useful way of testing physical assessment skills

Results “Excellent learning experience; I would be happy to do it again” “Great for learning clinical skills; for a mental health nurse this experience was very worth while” “I feel more confident now that I have participated in this” “I think this has given me more confidence in my clinical skills that I do not get to practice in a mental health setting” “although I found the scenario stressful I found it to be a valuable experience”

Challenges To offer simulation in both year 2 and year 3 of the nursing programme for all fields of nursing. To have shared learning in a simulation context across all fields of nursing in an undergraduate programme.
