4 Children’s Community Nursing (CCN)

5 Aims of the service To provide specialist nursing care or advice to any child/young person or family who require it, enabling the child/young person to be cared for at home. To prevent hospitalisation To facilitate seamless care from tertiary & secondary care to primary care

6 What does the CCN team do? The CCN team offers quality nursing care to children and young people within their own home. We also provide ongoing support to their families and training to any carers.

8 Some examples of what we do Injections Support/Liaison & Training Lead professional/key worker role Overnight oximetry Enteral feeding Continence TPN Central Line Management & IV antibiotics (once a day - bolus or short infusion only) Equipment supplies i.e. suction machines, nebulisers, syringes etc

9 Referral Criteria Children and Young People aged 0 – 19 years with a nursing need Open referral system

10 Current Caseload 120 children Neonatal Surgery, Respiratory, Cardiac, Rheumatology, Cerebral Palsy, Renal, Gastroenterology.

12 THE JIGSAW TEAM

13 Who are the Jigsaw Team? Set up as a joint initiative between Wakefield District Primary Care Trust and Wakefield Council, the Jigsaw team is made up of specialist staff who provide a package of integrated support services within the home. Initially funded for 3 years by Big Lottery.

14 What does the Jigsaw Team do? The Jigsaw Team offers quality, holistic care to children and young people who have a life-limiting or life-threatening illness. (Children’s Palliative Care) Wakefield District wide service

15 Aims of the service To enhance the quality of life for children and young people whilst also supporting their families and siblings To complement the existing services available for children and young people To be flexible, timely and responsive to any changing needs.

17 Community Children’s Nurses The Community Children’s Nurses provide holistic nursing care and support. Their involvement starts with diagnosis and follows through to symptom management, end of life care and bereavement support. This incorporates working in partnership with families and other support agencies.

18 Community Nursery Nurse The Community Nursery Nurse provides quality time and support to children and young people and their siblings in their own home. Through various play activities children come to understand their illness and ongoing treatment and providing distraction therapy.

19 Education Services Working in partnership with Pinderfields Hospital School

20 Bereavement Support Support and counselling are available for any family member coping with: the effects of diagnosis coming to terms with the emotional and practical changes to family life the grief and loss of bereavement readjustments in the ensuing months
