5 The Jefferson County Board of Education offers eligible employees the opportunity to save for retirement by participating in a 403(b) Plan. You are eligible to participate in this plan, whether or not you are actively contributing to it. In most cases, your deductions are “pre-taxed”. Not yet contributing to the 403(b) plan? To start your contributions to the 403(b) plan, complete and return a salary reduction agreement to Shonta Walker at the county office. You may enroll at any time but paperwork must be turned in during the first 2 weeks of the month you wish to begin contributing. Contributions will begin the first of the month after the paperwork is received. You must also establish an account with the appropriate investment provider(s) that you have selected. Already contributing the 403(b) plan? Great news! You have an opportunity to increase your contributions to the 403(b) Plan. If you are already currently contributing to the 403(b) plan, you may be able to increase your pre- tax contributions. To change your contributions, complete and return a salary reduction agreement to Shonta Walker. Need to reduce your contributions to the 403(b) Plan? You can change your contribution rate by completing and returning a salary reduction agreement as described above. Need to stop your contributions to the 403(b) Plan? You may stop contributing at any time with written notice to Shonta Walker. However, you must contact your agent to resume contributions. How much can I contribute? You may contribute up to $17,500 in 2013. This amount may be adjusted annually. Also, if you are at least 50 years old and/or you have completed at least 15 years of service, you may also be able to make additional catch-up contributions. 14 HEALTH CARE State Health Benefits Plan EMPLOYEE TOBACCO USERS CESSATION POLICY This policy applies to members who are tobacco users and paying the tobacco surcharge. Healthways’ offers telephonic tobacco cessation well-being coaching programs to assist SHBP members to quit using tobacco products. SHBP members who enroll and are actively engaged in the telephonic tobacco cessation well-being coaching programs are eligible for coverage of tobacco cessation medications. For complete details or to sign- up for the telephonic tobacco cessation well-being coaching, please call Healthways at: 1-888-616-6411. This policy allows you to have the tobacco surcharge removed by completing the tobacco surcharge removal requirements through your well-being vendor Healthways as outlined below. The maximum surcharge applies if the member or any of the member’s enrolled dependents are not tobacco-free or do not complete the telephonic tobacco cessation well-being coaching program. If the covered tobacco user (you and any of your covered dependents) completes the telephonic tobacco cessation well-being coaching program, you will be able to avoid the surcharge for the entire year. This means that all of the tobacco surcharge you paid in 2014 will be refunded after the completion all of the tobacco surcharge removal requirements. All tobacco surcharge removal requirements must be completed in 2014.

15 HEALTH CARE State Health Benefits Plan EMPLOYEE TOBACCO USERS CESSATION POLICY TOBACCO SURCHARGE REMOVAL REQUIREMENTS NOTE: Tobacco free means that you have not used tobacco products within the last 60 days. If you are Tobacco-Free for less than 60 days you are subject to the tobacco surcharge unless you do the following: You must: 1. Complete the online Healthways Well-Being Assessment in 2014 at BeWellSHBP.com by clicking “Take My Well- Being Assessment” to begin. 2. Once you complete your Well-Being Assessment within 24 hours you will receive an email from Healthways to create your Well-Being Plan. Please complete your Well-Being Plan. 3. Complete a telephonic tobacco cessation well-being coaching program through Healthways in 2014. Please call Healthways at 1-888-616-6411 to enroll. 4. Once you have completed Steps 1, 2 and 3 above, call Healthways at: 1-888- 616-6411, select the coaching option, and notify them that you have completed the requirements for tobacco surcharge removal and request a Letter of Completion to retain for your records. If you think you might be unable to complete the tobacco surcharge removal requirements, you might qualify for an opportunity to avoid the tobacco surcharge by different means. Contact Healthways at 1-888-616-6411 and we will work with you (and, if you wish, with your doctor) to find a well-being program with the same reward that is right for you in light of your health status.

16 HEALTH CARE State Health Benefits Plan Wellness in 2014 What to Do: 1.Assess Your Health Complete your Healthways Well-Being Assessment, confidential on-line questionnaire that takes will take you about 20 minutes *** Complete BOTH and earn $240 for your HRA account*** *** Wellness Biometric Assessment MUST be completed*** 2.Know Your Numbers Complete a biometric screening and submit test results (BMI, BP, Cholesterol, Glucose). Screening results must be completed at a SHPB-sponsored screening event or by your physician and your results submitted appropriately. 3.Take Action It’s your choice! Complete the coaching pathway, on-line pathway or both. Coaching Pathway Create your Well Being Plan Record 5 on-line well being activities using the same tracker with 4 consecutive weeks and earn $40 into your HRA account. You can earn these HRA dollars ($40) up to 6 times. Sample activities: track exercise, track daily steps, track food
