Hospital confinement indemnity insurance
6 6 Accident insurance – guaranteed issue available Accidents happen more often than you think. Statistics show that accidents happen where you and your family spend the most time: A work-related injury occurs every six seconds in the United States. Most injuries occurred in or around the home (52.6%) in 2009, followed by injuries at recreational and sport facilities (12.4%) and injuries on streets, highways, sidewalks and parking lots (11.1%). Injury Facts, National Safety Council, 2012 edition Accident insurance from Colonial Life can help prepare you for the unexpected. You are paid benefits to help you with the care and treatment of a covered accidental injury. Your benefits are paid directly to you, unless you specify otherwise. You are paid benefits regardless of any other insurance you may have. If your plan qualifies, you can take it with you if you change jobs or retire.
7 7 Accident insurance Benefit details Guaranteed issue (no health questions) for the base accident plan and the accident only disability riders Pays specific dollar amount benefit based on type of service Six plan choices with varying benefits and three optional riders Provides accidental and catastrophic benefit Benefits are paid directly to you Benefits are paid in addition to other insurance you may have Benefits are level for you, your spouse and your children except for accidental death and catastrophic accident benefits
9 9 Benefit details Two levels of coverage available (benefits vary by level chosen) Wellness benefit provided Three optional riders available: specified disease, initial diagnosis and progressive payment Benefits are paid directly to you Benefits are paid in addition to other insurance you may have Coverage is portable Spouse stand-alone plan coverage available Same premium, regardless of risk class or age, for all eligible employees (not age banded)
11 Critical illness insurance Benefit details Individual critical illness plan with subsequent diagnosis You can use the coverage more than once. Coverage includes age banded and tobacco/non-tobacco rates. Benefits are paid in addition to other insurance you may have. Coverage is portable. Benefits range from $5,000 - $100,000 for employee.

18 WellCard Savings Discount Program 2014 open enrollment Meet with a Colonial Life benefits counselor during open enrollment to receive a complimentary WellCard savings discount card. No purchase necessary to receive. Present the WellCard at any participating doctor or pharmacy, and start saving money. The WellCard is not insurance and is not intended to replace insurance. Your spouse and children can use this card.
