A poorly designed questionnaire will still be useless whether is delivered through Google forms, SurveyMonkey® or a piece of paper.  You should get to google forms, SurveyMonkey®, or paper only after you have carefully designed the survey and the items that will be in the questionnaire.

5. Cons. of Google forms (when compared to SurveyMonkey®  Does not give you the option to control that respondents only answer the questionnaire once.  Many respondents may worry about the anonymity and confidentiality of responses in a free service like Google.
